高能量激光结合Mulligan动态关节松动术治疗乳腺癌术后肩关节活动受限的效果观察

摘　　要：目的:探讨高能量激光结合Mulligan动态关节松动术治疗乳腺癌术后肩关节活动受限的临床效果。方法:选取2021年1月—2023年10月广东省人民医院赣州医院收治的86例乳腺癌术后肩关节活动受限患者,按随机数字表法分为两组,各43例。对照组行Mulligan动态关节松动术治疗,观察组在对照组基础上加用高能量激光治疗。比较两组疼痛评分、肩关节活动度、肩关节功能、炎症及致痛物质水平、生活质量。结果:治疗后,观察组视觉模拟评分法(VAS)评分低于对照组,肩关节内收、外展、前屈及后伸活动度均高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组Constant-Murley各维度评分及总分均高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组C反应蛋白(CRP)、前列腺素E2(PGE2)、P物质(SP)水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组生活质量各项评分均高于对照组,差异均有统计学意义(P<0.05)。结论:高能量激光联合Mulligan动态关节松动术可提高乳腺癌术后肩关节活动受限治疗效果,加快肩关节活动恢复,减轻关节功能障碍,以改善患者生活质量。Objective:To investigate the clinical effect of high energy laser combined with Mulligan dynamic joint mobilization in the treatment of limited mobility of shoulder joint after breast cancer surgery.Method:A total of 86 patients with limited mobility of shoulder joint after breast cancer surgery admitted to Guangdong Provincial People's Hospital Ganzhou Hospital from January 2021 to October 2023 were selected and divided into two groups according to random number table method,with 43 cases in each group.The control group was treated with Mulligan dynamic joint mobilization,and the observation group was treated with high energy laser on the basis of control group.Pain score,shoulder range of motion,shoulder function,inflammation and pain-causing substances,quality of life were compared between the two groups.Result:After treatment,the visual analogue scale(VAS)score of observation group was lower than that of control group,and the shoulder adduction,abduction,anterior flexion and posterior extension motion were higher than those of control group,the differences were statistically significant(P<0.05).After treatment,the Constant-Murley scores in each dimension and total score of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the levels of C reactive protein(CRP),prostaglandin E2(PGE2)and substance P(SP)in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,the scores of quality of life in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:High energy laser combined with Mulligan dynamic joint mobilization can improve the treatment effect of shoulder joint mobility restriction after breast cancer surgery,accelerate the recovery of shoulder joint mobility,reduce joint dysfunction,and improve the quality of life of patients.
