摘  要:目的探讨无创脑水肿动态监护仪在大鼠脑出血(ICH)后颅内压(ICP)预测中的作用,为临床无创脑水肿监测提供可靠的技术和理论指导。方法对ICH造模后雄性SD大鼠先行脑室内有创ICP监测,同时采用BORN-BE-Ⅲ无创脑水肿动态监护仪监测扰动系数(DC)。分析DC与脑水肿发展趋势之间规律,以及DC与ICP的相关性。结果建模第1~3天,ICP持续升高;第4~14天,ICP持续降低。建模后第1~3天DC有逐渐降低趋势,并于第3天降至最低点;第4~14天DC呈逐渐上升趋势;第10~14天DC逐渐回归至正常范围;第1~3天的DC变化幅度大于第4~14天。DC与ICP呈负相关。结论无创脑水肿动态监护仪可以监测ICH后脑水肿动态变化趋势,在ICP预测中发挥重要的参考作用;相较于颅内有创ICP监测技术,其具有安全、有效、连续动态床旁监护等优势,是ICP监测领域一项富有潜力的新型技术。Objective To explore the role of noninvasive dynamic monitor of cerebral edema in predicting intracranial pressure(ICP)after intracerebral hemorrhage(ICH)in rats,and to provide reliable technology and theoretical guidance for clinical non-invasive cerebral edema monitoring.Methods After ICH modeling,male SD rats were given intracerebroventricular invasive ICP monitoring,and the disturbance coefficient(DC)was monitored by BORN-BE-Ⅲnoninvasive dynamic monitor of cerebral edema.The law between DC and the development trend of cerebral edema and the correlation between DC and ICP were analyzed.Results ICP continued to rise from 1 to 3 days after modeling;from 4 to 14 days,ICP continued to decrease.DC decreased gradually from 1 to 3 days after modeling,and reached the lowest point on the third day;DC showed a gradual upward trend from 4 to 14 days;DC gradually returned to the normal range from 10 to 14 days;the amplitude of DC variation from 1 to 3 days was greater than that from 4 to 14 days.DC was negatively correlated with ICP.Conclusion The noninvasive dynamic monitor of cerebral edema can monitor the dynamic changes in cerebral edema after ICH and play an important reference role in ICP prediction;compared to invasive intracranial ICP monitoring technology,it has advantages such as safety,effectiveness,and continuous dynamic bedside monitoring,and is a potential new technology in the field of ICP monitoring.
