全腹腔镜下远端胃切除术联合三角吻合术治疗胃癌的效果及对胃肠蠕动功能的影响

摘　　要：目的探究全腹腔镜下远端胃切除术联合三角吻合术治疗胃癌的效果及对胃肠蠕动功能的影响。方法选择2019年7月至2022年12月我院收治的80例胃癌患者作为研究对象,以随机数字表法将其分为对照组和观察组,每组40例。对照组接受全腹腔镜下远端胃切除术联合管状吻合术治疗,观察组给予全腹腔镜下远端胃切除术联合三角吻合术治疗。比较两组的手术治疗效果。结果观察组的手术时间、肠鸣音恢复时间、肛门首次排气时间以及住院时间均短于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的胃泌素(GAS)、胃动素(MTL)水平均高于对照组,血管活性肠肽(VIP)水平均低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的白蛋白(ALB)、转铁蛋白(TRF)及血红蛋白(HB)水平均高于对照组,差异具有统计学意义(P<0.05)。结论全腹腔镜下远端胃切除术联合三角吻合术治疗胃癌患者可缩短其手术时间、肠鸣音恢复时间、肛门首次排气时间以及住院时间,促进胃肠蠕动功能恢复,改善营养状态,值得推广及应用。Objective To investigate the effect of total laparoscopic distal gastrectomy combined with delta-shaped anastomosis in the treatment of gastric cancer and its influence on gastrointestinal motility function.Methods A total of 80 patients with gastric cancer admitted in our hospital from July 2019 to December 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with total laparoscopic distal gastrectomy combined with tubular anastomosis,and the observation group was treated with total laparoscopic distal gastrectomy combined with delta-shaped anastomosis.The surgical treatment effects of the two groups were compared.Results The operation time,bowel sound recovery time,first anal exhaust time and hospitalization time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of gastrin(GAS)and motilin(MTL)in the observation group were higher than those in the control group,the level of vasoactive intestinal peptide(VIP)was lower than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of albumin(ALB),transferring(TRF)and hemoglobin(HB)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The total laparoscopic distal gastrectomy combined with delta-shaped anastomosis for gastric cancer patients can shorten the operation time,recovery time of bowel sounds,first anal exhaust time and hospitalization time,promote the recovery of gastrointestinal motility function and improve nutritional status,which is worthy of promotion and application.
