摘  要:目的探讨血清鳞状细胞癌抗原(SCCA)、神经元特异性烯醇化酶(NSE)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)水平与中晚期肺腺癌患者血管生成及预后的关系。方法选取100例中晚期肺腺癌患者和50例健康体检者,分别作为肺腺癌组和健康组,采用电化学发光法检测两组受试者血清SCCA、NSE、CYFRA21-1水平,采用免疫组化法检测肺腺癌组织中微血管密度(MVD)计数。采用Spearman相关分析法分析肺腺癌组患者血清SCCA、NSE、CYFRA21-1水平与MVD计数的相关性。随访1年,采用Cox回归模型分析中晚期肺腺癌患者预后的影响因素。结果肺腺癌组患者血清SCCA、NSE、CYFRA21-1水平均明显高于健康组,差异均有统计学意义(P﹤0.01)。肺腺癌组患者血清SCCA、NSE、CYFRA21-1水平与MVD计数均呈正相关(r=0.406、0.383、0.437,P﹤0.05)。多因素Cox回归分析结果显示,淋巴结转移、SCCA≥2.19 ng/ml、NSE≥36.48 ng/ml、CYFRA21-1≥6.57 ng/ml均是中晚期肺腺癌患者预后不良的独立危险因素(P﹤0.05)。结论肺腺癌患者血清SCCA、NSE、CYFRA21-1水平均较高,且均与MVD计数呈正相关。淋巴结转移、SCCA≥2.19 ng/ml、NSE≥36.48 ng/ml、CYFRA21-1≥6.57 ng/ml均是中晚期肺腺癌患者预后不良的独立危险因素。Objective To investigate the relationship between serum squamous cell carcinoma antigen(SCCA),neuron specific enolase(NSE),cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)and angiogenesis,prognosis of patients with advanced lung adenocarcinoma.Method A total of 100 patients with advanced lung adenocarcinoma and 50 healthy subjects were selected as lung adenocarcinoma group and healthy group,respectively.The serum levels of SCCA,NSE and CYFRA21-1 in the two groups were detected by electrochemiluminescence method,and microvessel density(MVD)count in lung adenocarcinoma tissues was detected by immunohistochemical method.Spearman correlation analysis was used to analyze the correlation between serum SCCA,NSE,CYFRA21-1 levels and MVD count in lung adenocarcinoma group.After 1 year of follow-up,Cox regression model was used to analyze the prognostic influence factors of patients with advanced lung adenocarcinoma.Result The serum levels of SCCA,NSE and CYFRA21-1 in lung adenocarcinoma group were significantly higher than those in healthy group,and the differences were statistically significant(P<0.01).Serum SCCA,NSE,CYFRA21-1 levels were positively correlated with MVD count in lung adenocarcinoma group(r=0.406,0.383,0.437;P<0.05).Multivariate Cox regression analysis showed that lymph node metastasis,SCCA≥2.19 ng/ml,NSE≥36.48 ng/ml,CYFRA21-1≥6.57 ng/ml were independent risk factors for poor prognosis in patients with advanced lung adenocarcinoma(P<0.05).Conclusion Serum levels of SCCA,NSE and CYFRA21-1 were higher in patients with lung adenocarcinoma,and were positively correlated with MVD count.Lymph node metastasis,SCCA≥2.19 ng/ml,NSE≥36.48 ng/ml,CYFRA21-1≥6.57 ng/ml were independent risk factors for poor prognosis in patients with advanced lung adenocarcinoma.
