血清癌胚抗原、胃泌素释放肽前体、细胞角质蛋白19片段抗原21-1及鳞状细胞癌抗原对早期肺癌的诊断价值

摘　　要：目的探讨血清癌胚抗原(CEA)、胃泌素释放肽前体(Pro-GRP)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)、鳞状细胞癌抗原(SCC-Ag)对早期肺癌的诊断价值。方法选取117例早期肺癌患者和93例肺部良性疾病患者,分别作为观察组和对照组。检测并比较两组患者的血清CEA、Pro-GRP、CYFRA21-1、SCC-Ag水平,采用Logistic回归模型分析早期肺癌发生的影响因素。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估血清CEA、Pro-GRP、CYFRA21-1、SCC-Ag对早期肺癌的诊断价值。结果观察组患者的血清CEA、Pro-GRP、CYFRA21-1、SCC-Ag水平均高于对照组,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,CEA、Pro-GRP、CYFRA21-1、SCC-Ag阳性表达均是早期肺癌发生的独立危险因素(P﹤0.05)。ROC曲线显示,血清CEA、Pro-GRP、CYFRA21-1、SCC-Ag诊断早期肺癌的AUC分别为0.800、0.664、0.763、0.799。结论早期肺癌患者血清CEA、Pro-GRP、CYFRA21-1、SCC-Ag水平均升高,且上述指标对早期肺癌均具有一定的诊断价值。Objective To explore the diagnostic value of serum carcinoembryonic antigen(CEA),pro-gastrin-releasing peptide(Pro-GRP),cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)and squamous cell carcinoma antigen(SCCAg)in early lung cancer.Method A total of 117 patients with early lung cancer and 93 patients with benign lung diseases were selected as observation group and control group,respectively.The serum CEA,Pro-GRP,CYFRA21-1 and SCCAg levels of the two groups were detected and compared,and the influencing factors of early lung cancer were analyzed by Logistic regression model.The receiver operating characteristic(ROC)curve was plotted,the area under the curve(AUC)was calculated,and the diagnostic value of serum CEA,Pro-GRP,CYFRA21-1 and SCC-Ag in early lung cancer were evaluated.Result The levels of serum CEA,Pro-GRP,CYFRA21-1 and SCC-Ag in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that CEA,Pro-GRP,CYFRA21-1 and SCC-Ag positive expression were independent risk factors for early lung cancer(P<0.05).ROC curve showed that the AUC of serum CEA,Pro-GRP,CYFRA21-1 and SCC-Ag in the diagnosis of early lung cancer were 0.800,0.664,0.763 and 0.799,respectively.Conclusion Serum CEA,Pro-GRP,CYFRA21-1 and SCC-Ag levels were all elevated in patients with early lung cancer,and these indexes had certain diagnostic value for early lung cancer.
