摘　　要：目的探讨彩色多普勒血流成像(CDFI)对小儿神经母细胞瘤(NB)的诊断价值。方法选取96例NB患儿作为NB组,91例非NB患儿作为对照组,比较两组患儿血流动力学参数(搏动指数、阻力指数、平均血流速度),以病理检查结果作为诊断金标准,分析CDFI对NB疾病和分期的诊断效能。结果CDFI诊断NB的灵敏度为91.67%(88/96),特异度为92.31%(84/91),准确度为91.98%(172/187),其诊断NB的结果与病理检查结果的一致性较好(Kappa=0.840)。NB组患儿搏动指数、阻力指数和平均血流速度均明显高于对照组,差异均有统计学意义(P﹤0.01)。CDFI诊断NB分期的准确度为90.91%(80/88),其诊断NB分期的结果与病理结果的一致性较好(Kappa=0.864)。结论CDFI诊断NB的准确度、灵敏度和特异度均较高,可有效诊断NB分期,NB患者搏动指数、阻力指数和平均血流速度均较高,可为临床诊断NB提供参考依据,具有较高的诊断价值。Objective To explore the diagnostic value of color Doppler flow imaging(CDFI)in pediatric neuroblastoma(NB).Method A total of 96 NB children were selected as NB group and 91 non-NB children as control group.The hemodynamic parameters(pulse index,resistance index,average blood flow velocity)of the two groups were compared.The result of pathological examination was used as the diagnostic gold standard,the diagnostic efficacy of CDFI on the disease and stage of NB were analyzed.Result The sensitivity of CDFI in the diagnosis of NB was 91.67%(88/96),specificity was 92.31%(84/91),accuracy was 91.98%(172/187),indicating a good consistency between CDFI diagnosis of NB and pathological examination result(Kappa=0.840).The pulse index,resistance index and average blood flow velocity in NB group were significantly higher than those in control group,the differences were statistically significant(P<0.01).The accuracy of CDFI in diagnosing NB stage was 90.91%(80/88),indicating a good consistency between CDFI diagnosis of NB stage and pathological results(Kappa=0.864).Conclusion CDFI has high accuracy,sensitivity and specificity in the diagnosis of NB,and can effectively diagnose the stage of NB.The pulse index,resistance index and average blood flow velocity of NB patients are high,which provides a reference for clinical diagnosis of NB and has high diagnostic value.
