摘 要:目的 探讨PI-RADS v2.1评分系统在前列腺癌(PCa)诊断中的有效性,并分析其与实验室标志物联合使用的方案及其与Gleason分级的相关性,为提高PCa影像诊断准确率提供依据。方法 收集行bp-MRI检查的341例患者资料,最终纳入经病理证实且临床影像资料完整的PCa 63例,其中有临床意义的PCa(csPCa)55例、前列腺增生(BPH)98例,将PCa/csPCa组的实验室检查资料、影像数据与BPH组进行对比分析。结果 PCa/csPCa组与BPH组在年龄、总前列腺特异性抗原(tPSA)、游离前列腺特异性抗原(fPSA)、游离前列腺特异性抗原/总前列腺特异性抗原(tPSA/fPSA)、前列腺体积(PV)、前列腺特异性抗原密度(PSAD)、前列腺成像报告和数据系统评分(PI-RADS)方面比较差异有统计学意义(P<0.05)。评分与Gleason分级呈中等相关性(r=0.681)。ROC曲线分析显示,PSAD和PI-RADS评分对PCa及csPCa的诊断效能高于其他指标,联合PSAD和评分后诊断效能进一步提高。结论 PI-RADS v2.1评分系统联合临床及生化指标在PCa的诊断中显示出良好的诊断效能,可提高诊断的一致性和准确性。Objective To investigate the effects of the PI-RADS v2.1 scoring system in the diagnosis of prostate cancer(PCa)and to analyze the scheme of its combined use with laboratory markers and its correlation with Gleason grading,providing a basis for improving the accuracy of PCa imaging diagnosis.Methods Data from 341 patients who underwent bp-MRI examinations were collected,and finally,63 cases of PCa including 55 cases of csPCa and 98 cases of BPH confirmed by pathology with complete clinical imaging data were included.The laboratory examination data and imaging data of the PCa/csPCa group were compared with those of the BPH group.Results There were statistical differences in age,PSA,PV,PSAD,and PI-RADS scores between the PCa/csPCa group and the BPH group(P<0.05).The score showed moderate correlation with Gleason grading(r=0.681).ROC curve analysis showed that PSAD and PI-RADS scores had higher diagnostic performance for PCa and csPCa than other indicators.The diagnostic performance was further improved after combining PSAD and PI-RADS scores.Conclusion The PI-RADS v2.1 scoring system combined with clinical and biochemical indicators shows good diagnostic performance in the diagnosis of PCa,which can improve diagnostic consistency and accuracy.
