摘  要:目的 通过初步探究腿伤克斯特菌基本的生物学特点及药物敏感性,为该菌的鉴定及临床合理用药提供指导和帮助。方法 按照年龄、性别、主要诊断等,对10例呼吸道标本进行统计分析。采用细菌培养、革兰染色和基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)仪进行菌株鉴定,通过基本的生化反应实验分析其生物学特性,通过VITEK 2全自动微生物鉴定药敏系统进行体外药物敏感性分析。结果 腿伤克斯特菌好发于有基础疾病且长期处于炎症刺激下的中老年男性患者。临床上易与其他细菌表现出混合感染。一般细菌培养结果表明:该菌在血平板及含万古霉素的巧克力平板上呈现灰白色,在麦康凯上为淡紫色,且菌落均呈现表面粗糙、迁徙生长的现象。镜下显示为革兰氏阴性球杆菌,经飞行质谱仪鉴定为腿伤克斯特菌。经综合分析该菌药敏结果,发现其对喹诺酮类抗生素表现为中介或耐药,对青霉素类、头孢类、酶抑制剂类、碳青霉烯类抗生素一般表现为敏感。结论 腿伤克斯特菌是一种革兰氏阴性球杆菌,常与其他菌造成混合感染,对临床不优先推荐使用喹诺酮类抗生素。Objective To explore the basic biological characteristics and the drug sensitivity of Kerstersia gyiorum,and provide guidance and help for the identification and the clinical rational use of the bacterium.Method According to age,gender,and main diagnosis,10 respiratory specimens from clinical sources were statistically analyzed.Strain identification was performed by bacterial culture,Gram staining,and matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOF MS),and its biological characteristics were analyzed by basic biochemical reaction experiments.In vitro drug susceptibility analysis was performed by the VITEK 2 fully automated microbial identification and antimicrobial susceptibility system.Result Kerstersia gyiorum was more common in middle-aged and elderly men with underlying diseases and long-term inflammation.Clinically,it was easy to co-infect with other bacteria.The general bacterial culture results showed that the bacteria were grayish-white on blood plates and chocolate plates containing vancomycin,and light purple on MacConkey plates,and the colonies showed rough surface and migratory growth.Microscopically it was Gram-negative cocci and identified by MALDI-TOF MS as Kerstersia gyiorum.After comprehensive analysis of the drug susceptibility results of the bacterium,it was intermediate or resistant to quinolone antibiotics,and sensitive to penicillins,cephalosporins,enzyme inhibitors,and carbapenems.Conclusion Kerstersia gyiorum is a Gram-negative coccus bacterium and often co-infected with other bacteria.Quinolone antibiotics are not firstly recommended in clinical for this bacterium.
