摘 要:目的探讨子宫内膜癌(EC)组织中微小RNA-145-5p(miR-145-5p)、胰岛素样生长因子2 mRNA结合蛋白1(IGF2BP1)mRNA表达变化及与患者预后的关系。方法选取2018年5月—2021年5月于南通市肿瘤医院行手术切除的EC组织及对应癌旁组织标本115例份,采用实时荧光定量PCR法检测EC组织及癌旁组织中miR-145-5p、IGF2BP1 mRNA表达,Pearson相关法分析miR-145-5p与IGF2BP1 mRNA表达的相关性。根据EC组织中miR-145-5p、IGF2BP1 mRNA表达的中位数将患者分为高/低表达组,Kaplan-Meier法绘制高/低miR-145-5p、IGF2BP1 mRNA表达的EC患者生存曲线;多因素Cox回归分析影响EC患者预后的因素。结果与癌旁组织比较,EC组织中miR-145-5p表达降低,IGF2BP1 mRNA表达升高(t分别为-22.201、19.038,P均<0.05)。miR-145-5p与IGF2BP1 mRNA表达呈负相关(r=-0.748,P<0.05)。EC组织中miR-145-5p、IGF2BP1 mRNA表达与组织分化程度、国际妇产科联盟(FIGO)分期和淋巴结转移有关(P均<0.05)。115例EC患者3年总生存率为67.83%(78/115)。miR-145-5p高表达组3年总生存率高于miR-145-5p低表达组,IGF2BP1 mRNA高表达组3年总生存率低于IGF2BP1mRNA低表达组(χ^(2)分别为13.484、11.755,P均<0.05)。EC患者死亡的独立危险因素为组织分化程度低分化、FIGO分期Ⅲ期、有淋巴结转移、IGF2BP1 mRNA表达≥1.53(OR分别为5.434、6.092、4.327、5.339,95%CI分别为1.735~17.021、1.930~19.225、1.424~13.154、1.515~18.811),独立保护因素为miR-145-5p表达≥0.74(OR=0.298,95%CI:1.515~18.811)。结论EC组织中miR-145-5p低表达,IGF2BP1 mRNA高表达,与组织分化程度、FIGO分期、淋巴结转移和患者预后有关。Objective To investigate the expression changes of microRNA-145-5p(miR-145-5p)and insulin-like growth factor 2 mRNA-binding protein 1(IGF2BP1)in the endometrial carcinoma(EC)tissues and their relationship with the prognosis of patients.Methods Totally 115 specimens of EC tissues and corresponding adjacent tissues surgically resected in Nantong Tumor Hospital from May 2018 to May 2021 were selected,and real-time fluorescence quantitative PCR was used to detect the expression of miR-145-5p and IGF2BP1 mRNA in EC tissues and adjacent tissues.Pearson correlation was used to analyze the correlation between miR-145-5p and IGF2BP1 mRNA expression.According to the median miR-145-5p and IGF2BP1 mRNA expression in EC tissues,we divided the patients into the high/low expression groups.Kaplan-Meier method was used to plot the survival curves of EC patients with high/low miR-145-5p and IGF2BP1 mRNA expression;multifactor Cox regression was used to analyze the factors affecting the prognosis of EC patients.Results Compared with the adjacent tissues,miR-145-5p expression decreased and IGF2BP1 mRNA expression increased in EC tissues(t=–22.201,19.038,both P<0.05).The miR-145-5p was negatively correlated with IGF2BP1 mRNA expression(r=–0.748,P<0.05).In the EC tissues,miR-145-5p and IGF2BP1 mRNA expression was related to degree of differentiation,federation international of gynecology and obstetrics(FIGO)staging,and lymph node metastasis(all P<0.05).The 3-year overall survival rate of 115 EC patients was 67.83%(78/115).Patients with high miR-145-5p expression had a higher 3-year overall survival rate than patients with low miR-145-5p expression,and the 3-year overall survival rate of patients with high expression of IGF2BP1 mRNA was lower than that of patients with low expression of IGF2BP1 mRNA(χ^(2)=13.484,11.755,all P<0.05).The independent risk factors for death in EC patients were poor differentiation,FIGO stage III,lymph node metastasis,and IGF2BP1 mRNA≥1.53(OR=5.434,6.092,4.327,and 5.339;95%CI:1.735-17.021,1.930-19.225
