摘　　要：目的比较AH-Plus根管充填材料与iRoot SP根管充填材料在慢性根尖周炎患者根管治疗术中的应用效果。方法前瞻性研究,选取2021年5月至2022年10月在延安大学附属医院牙体牙髓病科治疗的慢性根尖周炎患者90例作为研究对象,利用随机数字表法分为对照组(45例)和观察组(45例)。对照组男24例、女21例,年龄(28.14±6.02)岁,病程(3.26±0.92)个月,接受根管治疗术时应用AH-Plus根管充填材料联合热牙胶垂直加压填充;观察组男22例、女23例,年龄(26.55±5.78)岁,病程(3.45±0.79)个月,接受根管治疗术时应用iRoot SP根管充填材料联合热牙胶垂直加压填充。对比两组患者疗效、疼痛评分[视觉模拟疼痛评分(VAS)]、治疗前后咀嚼功能及治疗满意度。统计学方法采用t检验、χ^(2)检验。结果观察组治疗总有效率为97.78%(44/45),高于对照组的73.33%(33/45),差异有统计学意义(χ^(2)=10.879,P<0.001)。观察组术后12、24、72 h的VAS评分均低于对照组[(1.52±0.19)分比(2.35±0.26)分、(2.65±0.31)分比(3.80±0.29)分、(3.29±0.20)分比(4.15±0.23)分],差异均有统计学意义(t=15.142、18.173、18.928,均P<0.05)。治疗前,两组患者咀嚼效率比较,差异无统计学意义(P>0.05);治疗后,观察组咀嚼效率高于对照组[(85.21±11.22)%比(72.76±10.23)%],差异有统计学意义(t=5.500,P<0.05)。观察组治疗总满意度为95.56%(43/45),高于对照组的77.78%(35/45),差异有统计学意义(χ^(2)=6.154,P=0.013)。结论与AH-Plus根管充填材料相比,iRoot SP根管充填材料在慢性根尖周炎患者根管充填治疗中表现出更好的治疗效果,可降低术后疼痛感,改善咀嚼功能,同时患者治疗满意度也有显著提升。Objective To compare the therapeutic effects of using epoxy resin-based canal sealer(AH-Plus)versus bioceramic iRoot SP in the root canal treatment for patients with chronic apical periodontitis.Methods Ninety patients with chronic apical periodontitis treated in Department of Conservative Dentistry and Endodontics,Hospital Affiliated to Yan'an University from May 2021 to October 2022 were selected for the randomized controlled trial,and were divided into a control group and an observation group by the random number table method,with 45 cases in each group.There were 24 males and 21 females in the control group;they were(28.14±6.02)years old;their disease course was(3.26±0.92)months.There were 22 males and 23 females in the observation group;they were(26.55±5.78)years old;their disease course was(3.45±0.79)months.The control group took root canal treatment with AH-Plus sealer and warm vertical compaction of gutta-percha,while the observation group were treated with iRoot SP sealer and took warm vertical compaction of gutta-percha.The efficacies,pain scores[Visual Analog Scale(VAS)],masticatory function(masticatory efficiency and absorption values in masticatory tests)before and after the treatment,and patient satisfaction were compared between the two groups.t andχ^(2) tests were applied.Results The total effective rate in the observation group was higher than that in the control group[97.78%(44/45)vs.73.33%(33/45)],with a statistical difference(χ^(2)=10.879;P<0.001).The scores of VAS 12,24,and 72 h after the treatment in the observation group were lower than those in the control group(1.52±0.19 vs.2.35±0.26,2.65±0.31 vs.3.80±0.29,and 3.29±0.20 vs.4.15±0.23),with statistical differences(t=15.142,18.173,and 18.928;all P<0.05).There was no statistical difference in the mastication efficiency between the two groups before the treatment(P>0.05).After the treatment,the mastication efficiency in the observation group was higher than that in the control group[(85.21±11.22)%vs.(72.76±10.23)%],with a statist
