颌骨囊肿摘除术联合同期引导骨再生技术修复术后骨缺损的临床疗效分析

摘　　要：目的:探讨颌骨囊肿摘除术联合同期引导骨再生技术(Guided bone regeneration,GBR)对颌骨囊肿患者的疗效及对骨缺损的修复作用。方法:选取2018年1月-2022年12月安徽医科大学第一附属医院收治的60例颌骨囊肿患者,根据其治疗方法将其分为研究组(30例)及对照组(30例),对照组采用颌骨囊肿摘除术进行治疗,研究组采用颌骨囊肿摘除术联合同期GBR技术治疗。采用锥形束CT(Cone beam CT,CBCT)影像学参数评估患者骨缺损修复情况,比较两组术后3个月、6个月的囊肿体积缩小量、囊肿面积缩小量、骨密度、骨质增生厚度、颌骨高度及宽度;比较两组患者的临床疗效。结果:两组患者术后3个月及术后6个月CT值均高于术前(P<0.05),术后6个月均高于术后3个月(P<0.05),且研究组术后3个月及术后6个月CT值高于对照组(P<0.05);两组术后6个月囊肿体积缩小量及囊肿面积缩小量均大于术后3个月(P<0.05),且研究组术后3个月及术后6个月囊肿体积缩小量及囊肿面积缩小量均大于对照组(P<0.05);两组术后6个月骨密度及骨质增生厚度均高于术后3个月(P<0.05),且研究组术后3个月及术后6个月的骨密度、骨质增生厚度均高于对照组(P<0.05);两组术后6个月上下颌骨高度及宽度均高于术后3个月(P<0.05),且研究组术后6个月上下颌骨高度及宽度均高于对照组(P<0.05);研究组临床有效率高于对照组(P<0.05)。结论:颌骨囊肿摘除术联合同期GBR技术能够显著修复骨缺损,有效缩小囊肿体积及面积,促进颌骨生长发育,疗效显著。Objective To explore the curative effect of jaw cyst excision combined with simultaneous guided bone regeneration(GBR)on patients with jaw cyst and the repair of bone defect.Methods 60 patients with jaw cysts who were treated in First Affiliated Hospital of Anhui Medical University from January 2018 to December 2022,and divide them into study group(30 cases)and control group(30 cases)according to their treatment methods.The control group was treated with removal of jaw cyst,while the study group was treated with removal of jaw cyst combined with GBR technology at the same time.Evaluation of bone defect repair in patients with cone-beam CT(CBCT)imaging parameters.The reduction of cyst volume,reduction of cyst area,bone density,thickness of bone hyperplasia,height and width of jaw bone in the two groups were compared 3 and 6 months after operation.The clinical efficacy of the two groups were compared.Results The CT values of patients in the two groups were higher than those before operation at 3 months and 6 months after operation(P<0.05),Six months after operation were higher than three months after operation(P<0.05),The CT value of the study group was higher than that of the control group at 3 months and 6 months after operation(P<0.05).The reduction of cyst volume and area in both groups at 6 months after operation was greater than that at 3 months after operation(P<0.05),The reduction of cyst volume and area in the study group were greater than those in the control group at 3 months and 6 months after operation(P<0.05).The bone mineral density and thickness of bone hyperplasia in the two groups at 6 months after operation were higher than those at 3 months after operation(P<0.05),and the bone mineral density and thickness of bone hyperplasia in the study group at 3 months and 6 months after operation were higher than those in the control group(P<0.05).The height and width of maxilla and mandible in the two groups at 6 months after operation were higher than those at 3 months after operation(P<0.05),In addition,
