不同病变长径的早期食管癌患者经内镜下黏膜剥离术治疗的疗效及并发症比较

摘　　要：目的探讨不同病变长径的早期食管癌患者经内镜下黏膜剥离术治疗的疗效及并发症比较。方法收集采用经内镜下黏膜剥离术治疗的早期食管癌病变长径<3 cm的早期食管癌患者65例为对照组,收集同期病变直径>3 cm经内镜下黏膜剥离术的早期食管癌患者85例为试验组。对比两组患者的病灶切除效果,围术期情况,治疗前后3个月的血清肿瘤标志物水平及两组患者的并发症发生率。结果两组患者的整块切除率和完全切除率对比无差异(P>0.05);试验组患者的手术时间、术后禁食时间长于对照组(P<0.05),抗生素使用时间、住院时间对比无差异(P>0.05);治疗3个月后,两组患者的CEA、SCC-Ag、CA19-9的水平均较治疗前低(P<0.05),但两组患者的CEA、SCC-Ag、CA19-9水平对比无差异(P>0.05);试验组患者的并发症发生率较对照组高(P<0.05)。结论经内镜下黏膜剥离术治疗不同病变长径的早期食管癌患者的切除效果、3个月后的肿瘤标志物水平、抗生素使用时间和住院时间相近,病变长径较长者的手术时间、术后进食时间以及并发生发生率略高于病变长径较短者,具有临床参考意义。Objective To explore and compare the efficacy and complications of endoscopic mucosal dissection in patients with early esophageal cancer with different lesion lengths.Methods 65 patients with early esophageal cancer with lesion length<3 cm who underwent endoscopic mucosal dissection were collected as the control group,and 85 patients with early esophageal cancer with lesion length>3 cm were included in the test group.The lesion resection effect,perioperative condition,serum tumor markers before treatment and at 3 months after treatment and incidence rates of complications were compared between the 2 groups.Results There were no differences in the en bloc resection rate and total resection rate between groups(P>0.05).The surgical time and postoperative fasting time in test group were longer than those in the control group(P<0.05),but there were no differences in antibiotic use time and hospital stay(P>0.05).After 3 months of treatment,the levels of CEA,SCC-Ag and CA19-9 in both groups were reduced compared with those before treatment(P<0.05),but there were no differences in the levels of CEA,SCC-Ag and CA19-9 between both groups(P>0.05).The incidence rates of complications were higher in test group than those in the control group(P<0.05).Conclusion Endoscopic mucosal dissection has similar resection effect,tumor markers levels after 3 months,anti biotic use time and hospital stay in patients with early esophageal cancer with different lesion lengths.The surgical time,postoperative feeding time and incidence rates of complications of patients with longer length of lesion are slightly longer or higher than those of patients with shorter length of lesion,which has clinical significance.
