摘　　要：目的:统计武汉市武昌区2022年11 826例适龄妇女宫颈癌筛查项目中人乳头状瘤病毒(HPV)感染情况,分析本地区适龄妇女HPV感染的流行学特征及其与子宫颈上皮内病变的关系。方法:选取武汉市武昌区2022年1月1日至8月7日接受免费宫颈癌筛查的11 826名适龄妇女作为对象,同时进行人工智能宫颈液基细胞学检查(TCT)、人乳头瘤病毒(HPV)检测,初筛阳性患者进行组织病理学检查。采用SPSS 27.0对HPV感染亚型检出率、不同年龄HPV感染亚型分布、不同亚型HPV感染与子宫颈上皮内病变程度的关系等进行统计学分析。结果:武昌区11 826名适龄妇女宫颈癌筛查HPV感染率为10.37%(1 226/11 826),15种HPV亚型中,感染检出率最高的前3位分别是HPV52(3.26%)、HPV58(1.46%)、HPV16(1.21%)。武昌地区HPV检出率最高的年龄段为55~59岁,其次为30~34岁,感染HPV亚型前3位分别是52、58、16型。根据筛查程序共有487名妇女进行了阴道镜及子宫颈病理学检查,其中宫颈癌及癌前病变患者59人,HPV亚型感染率前3位分别是HPV16(45.76%)、HPV52(23.73%)、HPV58(15.25%)。宫颈癌及癌前病变患者中,多重HPV感染发生率7.75%(10/129),是单一HPV感染发生率4.47%(49/1 097)的1.73倍。结论:武昌区适龄妇女HPV感染率10.37%,亚型检出率前3位分别是HPV52、58、16型,HPV感染发生的高峰年龄段为55~59岁及30~34岁,上述年龄段HPV感染的主要亚型为HPV52、58、16型,这3种亚型在高级别子宫颈上皮内病变患者中的检出率也最高,与既往国外报道的HPV18亚型是最常见致癌亚型略有不同,宫颈癌及癌前病变患者中,多重HPV感染发生率是单一HPV感染发生率的1.73倍。Objective:To retrospectively analyze cervical screening data from 11826 women of appropriate age in Wuchang in 2022, to determine the prevalence of human papillomavirus (HPV) in the populationof Wuchang District, Wuhan, China, and to further analyze the possible relationship betweenHPV and cervical lesions. Methods: A total of 11 826 women aged 30-65 years in Wuchang District,Wuhan City in January 1, 2022 to August 7, 2022, were selected for HPV detection by artificial intelligencecytological examination (TCT) and cervical cancer screening. SPSS 27. 0 was used to analyzethe detection rate of HPV infection subtypes, distribution of HPV subtypes at different ages, and therelationship between HPV subtypes and the degree of cervical intraepithelial lesions. Results: TheHPV infection rate was 10. 37%(1 226/11 826). Among the 15 HPV subtypes, the top three with thehighest infection rate were HPV52 (3. 26%), HPV58 (1. 46%), and HPV16 (1. 21%). The agegroup with the highest HPV detection rate was 55-59 years old group, followed by 30-34 years oldgroup, and the top three with the highest infection detection rate were HPV52, HPV58, andHPV16. According to the screening process, a total of 487 women underwent colposcopy and cervicalpathology examinations, among whom 59 were diagnosed with cervical cancer and precancerous lesions.The top three HPV subtypes with the highest infection rate were HPV16 (45. 76%), HPV52(23. 73%), and HPV58 (15. 25%), respectively. In patients with cervical cancer and precancerous lesions,the incidence of multiple HPV infection was 7. 75% (10/129), which was 1. 73 times higherthan the incidence of single HPV infections at 4. 47% (49/1 097). Conclusion: The HPV infectionrate in Wuchang District was 10. 37%, the top three with the highest infection detection rate wereHPV52, HPV58, and HPV16.
