摘　　要：目的:探讨新辅助化疗联合腹腔镜根治术治疗进展期胃癌的效果。方法:选取本院2018年8月—2023年6月收治的进展期胃癌患者117例,根据治疗方法不同分为两组,其中对照组58例接受腹腔镜胃癌根治术,观察组59例先行新辅助化疗后接受腹腔镜胃癌根治术。比较两组手术相关参数、免疫功能指标、PGⅡ、MMP-2、MMP-9、VEGF-A水平及并发症。结果:术后,观察组、对照组中转开腹率、并发症发生率清扫淋巴结个数、术中出血量、术后排气时间、手术时间、拔出引流管时间比较,差异无统计学意义(P>0.05);观察组CD3^(+)、CD4^(+)水平高于对照组,Ig A、Ig E、PGⅡ、MMP-2、MMP-9、VEGF-A水平低于对照组(P<0.05)。结论:新辅助化疗联合腹腔镜根治术治疗可以有效改善进展期胃癌患者免疫功能,调节PG、MMP、VEGF表达,且不会对手术相关参数和并发症产生明显影响,安全性较高。Objective:To investigate the effect of neoadjuvant chemotherapy combined with laparoscopic radical resection in the treatment of advanced gastric cancer.Methods:A total of 117 patients with advanced gastric cancer from August 2018 to June 2023 were selected and divided into two groups according to different treatment methods.Among them,58 patients of the control group received laparoscopic radical gastrectomy,and 59 patients of the observation group received laparoscopic radical gastrectomy after neoadjuvant chemotherapy.The operation-related parameters,immune function indexes,PGⅡ,MMP-2,MMP-9,VEGF-A levels and complications were compared between the two groups.Results:After operation,there was no significant difference in the rate of conversion to laparotomy,the incidence of complications,the number of lymph nodes dissected,intraoperative blood loss,postoperative exhaust time,operation time and drainage tube removal time between the observation group and the control group(P>0.05).The levels of CD3^(+)and CD4^(+)in the observation group were higher than those in the control group,and the levels of IgA,Ig E,PGII,MMP-2,MMP-9 and VEGF-A were lower than those in the control group(P<0.05).Conclusion:Neoadjuvant chemotherapy combined with laparoscopic radical gastrectomy can effectively improve the immune function of patients with advanced gastric cancer,regulate the expression of PG,MMP and VEGF,and have no obvious influence on operation-related parameters and complications,so it is safe.
