巨淀粉酶血症2例并文献复习

摘　　要：巨淀粉酶血症是一种罕见的疾病,以血淀粉酶升高为标志,无特异性症状和体征。巨淀粉酶是由淀粉酶和免疫球蛋白组成的大分子循环复合物不能经肾脏滤过而产生。本研究报道2例巨淀粉酶血症,血淀粉酶升高,肾功能正常,尿淀粉酶正常,淀粉酶肌酐清除率小于1%,聚乙二醇(polyethylene glycol,PEG)沉淀率大于73%,排除胰腺、腮腺疾病及其他相关疾病所致血淀粉酶升高,诊断为巨淀粉酶血症。巨淀粉酶血症不仅与自身免疫性疾病、恶性肿瘤等疾病相关,在健康人群中也可发生。Macroamylasemia is a rare disease marked by elevated blood amylase without specific symptoms and signs.Macroamylase is a macromolecular circulating complex of amylase and immunoglobulin that cannot be filtered through the kidney.Two cases of macroamylasemia were reported,with elevated serum amylase,normal renal function,normal urine amylase,amylase-creatinine clearance rate of less than 1%,and polyethylene glycol(PEG)precipitation rate of more than 73%.Pancreatic and parotid gland diseases and other related diseases were excluded.Macroamylasemia is not only associated with autoimmune diseases,malignant tumors and other diseases,but also can occur in healthy people.
