摘 要:目的调查四川省三级甲等医院外科医务人员工作不安全感、情绪调节策略与睡眠质量现状及其相关性,探讨工作不安全感、情绪调节策略对睡眠质量的影响。方法采用横断面研究设计,于2023年8月选取四川省5所三级甲等医院308名外科医务人员为调查对象,采用工作不安全感量表、情绪调节策略量表和睡眠质量量表进行调查,分析三级甲等医院外科医务人员睡眠质量的相关影响因素。结果308名外科医务人员匹兹堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)量表得分为(8.34±4.56)分,其中241名(78.25%)外科医务人员PSQI总分≥5分,存在不同程度的睡眠障碍;工作不安全感得分为(14.84±3.80)分;情绪调节策略得分为(45.31±12.08)分。单因素分析显示,不同性别、年龄、婚育状况、教育水平、岗位类别、职称、是否轮值夜班、运动锻炼频率(次/周)、健康状况、是否有慢病、参加心理减压培训、是否饮咖啡及是否饮茶的外科医务人员睡眠质量总分比较,差异均有统计学意义(均P<0.05)。相关性分析显示,外科医务人员PSQI总分及各维度得分与工作不安全感总分及各维度得分均呈正相关(r=0.317~0.675,均P<0.05);与情绪调节策略总分及各维度得分均呈负相关(r=-0.241~-0.684,均P<0.05)。多因素分析显示,是否轮值夜班、运动锻炼频率、是否有慢性病、心理减压课程培训、工作不安全感及情绪调节策略均是急诊科医务人员睡眠质量的主要影响因素(均P<0.05),共解释总变量的66.6%。结论四川省5所三级甲等医院外科医务人员睡眠质量不佳,大部分医务人员存在不同程度睡眠障碍,其中工作不安全感是睡眠质量的危险因素,情绪调节策略是其重要保护性因素。医院管理者应重视医务人员睡眠健康状况,降低其工作不安全感,提升其情绪调节策略,从而改善其睡眠质量。Objective To investigate the current situation and correlation of job insecurity,emotion regulation strategies and sleep quality of surgical medical staff in grade-A tertiary hospitals,and to explore the influence of job insecurity and emotion regulation strategies on sleep quality.Methods A cross-sectional study design was adopted to select 308 surgical medical staff from 5 gradeA tertiary hospitals in Sichuan Province as the survey subjects in August 2023.The work insecurity scale,emotion regulation strategy scale,and sleep quality scale were used for the survey to analyze the relevant influencing factors of sleep quality among surgical medical staff in grade-A tertiary hospitals.Results The Pittsburgh sleep quality index(PSQI)score of 308 surgical medical staff was(8.34±4.56)points,of which 241(78.25%)surgical medical staff had a total PSQI score of≥5 points,with different degrees of sleep disorders.The score of job insecurity was(14.84±3.80)points,the score of emotion regulation strategy was(45.31±12.08)points.Univariate analysis showed that there were statistically significant differences in the total score of sleep quality among surgical medical staff with different gender,age,marital status,education level,job category,professional title,night shift rotation,frequency of exercise(times/week),health status,presence of chronic disease,participation in psychological decompression training,coffee and tea consumption(all P<0.05).Correlation analysis showed that the total score and each dimension score of PSQI of surgical medical staff were positively correlated with the total score and each dimension score of job insecurity(r=0.317-0.675,all P<0.05),and negatively correlated with the total score and each dimension score of emotion regulation strategy(r=-0.241--0.684,all P<0.05).Multivariate analysis showed that night shift rotation,frequency of exercise,presence of chronic diseases,psychological decompression course training,job insecurity and emotion regulation strategies were the main influencing factors
