中医透窍解毒法论治前列腺癌的科学内涵

摘　　要：本文全面探讨了中医透窍法在前列腺癌(PCa)治疗中的应用,并基于国医大师周仲瑛提出的前列腺癌“癌毒”病机理论和透窍法因势利导的特性,创新提出了“透窍解毒”治疗思路。此疗法主要作用于中医理论中的“精窍”或“溺窍”,运用芳香透窍药物作为引经药,促进了清解癌毒药物穿透前列腺上皮屏障并发挥药效,同时有助于瘀毒从窍道排出。进一步分析了《外科证治全生集》中的西黄丸在PCa治疗中的潜在应用机制。通过现代网络药理学和体内外实验,明确了西黄丸通过多通路、多靶点发挥的抑癌作用,如调控前列腺上皮屏障通透性、抑制炎症免疫反应、缓解前列腺疼痛、抑制PCa细胞增殖及诱导细胞凋亡等。从现代医学角度揭示了透窍解毒法论治PCa的科学内涵,以期为PCa的治疗提供更多有效的选择。This article comprehensively discusses the application of the TCM penetration method in the treatment of prostate cancer(PCa),and proposes an innovative treatment idea of“penetrating and detoxifying”based on national medical master Zhou Zhongying'cancer toxin pathogenesis theory of prostate cancer and the characteristics of the penetration method.This therapy mainly acts on the“essence orifice”or“urinary orifice”in traditional Chinese medicine theory,and uses aromatic penetration drugs as a guiding medicine to promote the penetration of cancer toxin-clearing drugs through the prostate epithelial barrier and exerts their efficacy,while also helps to discharge stasis and toxins from the orifice.We further analyze the potential application mechanism of Xihuang Pill in the treatment of PCa in the“Complete Collection of Surgical Diagnosis and Treatment”.Through modern network pharmacology and in vitro and in vivo experiments,it is clarified that Xihuang Pill exerts its anticancer effects through multiple pathways and multiple targets,such as regulating the permeability of the prostate epithelial barrier,inhibiting inflammatory immune response,alleviating prostate pain,inhibiting PCa cell proliferation,and inducing cell apoptosis.From a modern medical perspective,it reveals the scientific connotation of the theory of“penetrating and detoxifying”in the treatment of PCa for more effective treatment options for PCa.
