3D CUBE T_(1)序列与SWI序列对急性缺血性脑卒中患者预后的评估价值

摘　　要：【目的】探讨高分辨率磁共振管壁成像(HR-MRI)3D CUBE T_(1)序列与SWI序列信号征象对急性缺血性脑卒中(AIS)患者预后的评估价值。【方法】选择2019年7月至2021年7月西安凤城医院收治的84例急性缺血性脑卒中患者的临床资料,根据治疗3个月后的Rankin量表评分分为预后不良组、预后良好组。比较两组AIS患者的一般资料、HR-MRI不同序列图像,采用Logistic回归分析AIS患者预后的影响因素,根据受试者工作特征(ROC)曲线分析3D CUBE T_(1)序列与SWI序列信号征象对AIS预后的评估价值。【结果】预后不良组病变部位3D CUBE T_(1)序列呈高信号征象,SWI序列呈低信号征象及NIHSS评分>15分患者占比高于预后良好组(P<0.05)。Logistic回归分析结果显示,患者病变部位3D CUBE T_(1)序列高信号征象、SWI序列低信号征象及NIHSS评分>15分是AIS患者预后不良的独立危险因素(P<0.05)。ROC曲线分析显示,3D CUBE T_(1)序列呈现高信号征象、SWI序列呈现低信号征象评估AIS预后的曲线下面积(AUC)分别为0.758、0.778,二者联合预测的AUC值为0.885。【结论】HR-MRI 3D CUBE T_(1)序列呈现高信号征象与SWI序列呈现低信号征象对于评估AIS患者预后均有一定的价值,二者联合对于AIS患者预后的评估价值更高。【Objective】To explore the prognostic value of high-resolution magnetic resonance imaging(HR-MRI)3D CUBE T_(1)sequence and SWI sequence in patients with acute ischemic stroke(AIS).【Methods】The clinical data of 84 patients with AIS admitted to our hospital from July 2019 to July 2021 were collected.According to the Rankin scale score after 3 months of treatment,the patients were divided into a poor prognosis group and a good prognosis group.All patients were examined by HR-MRI 3D CUBE T_(1)sequence and SWI sequence,and the general data and HR-MRI images of different sequences of AIS patients were compared between the two groups.Logistic regression was used to analyze the prognostic factors of AIS patients,and the ROC curve was used to analyze the evaluation value of 3D CUBE T_(1)sequence and SWI sequence signal signs in the prognosis of AIS.【Results】The proportion of patients with high signal on 3D CUBE T_(1)sequence,low signal on SWI sequence and NIHSS score>15 in the poor prognosis group was higher than that in the good prognosis group(P<0.05).Logistic regression analysis showed that 3D CUBE T_(1)sequence high signal,SWI sequence low signal and NIHSS score>15 were independent risk factors for poor prognosis of AIS patients(P<0.05).The ROC analysis showed that the AUC values of 3D CUBE T_(1)sequence,SWI sequence to evaluate the prognosis of AIS were 0.758 and 0.778,respectively.The AUC value of the joint prediction(3D CUBE T_(1)and SWI combination)was 0.885.【Conclusion】Both the hyperintense signs of HR-MRI 3D CUBE T_(1)sequence and the hypointense signs of SWI sequence have certain value for evaluating the prognosis of AIS patients,while the combination of the two has higher value for evaluating the prognosis of AIS patients.
