内镜CSP与HSP治疗直径≤10mm结直肠息肉患者中的效果对比

摘　　要：目的:比较分析内镜圈套器冷切除术(CSP)与内镜圈套器热切除术(HSP)治疗直径≤10mm结直肠息肉(CP)患者的效果。方法:收集本院2021年10月~2024年1月180例直径≤10mmCP患者,依照手术方式不同分为CSP组90例接受内镜CSP治疗,HSP组90例接受内镜HSP治疗,比较两组治疗效果。结果:CSP组息肉切除时间、钛夹使用数、治疗费用均少于HSP组(P<0.05),组间息肉完全切除率、息肉完整切除率、息肉回收率无明显差异(P>0.05);CSP组术中出血发生率较HSP组高,迟发性出血、黏膜下层累及损伤、穿孔、电凝综合征发生率均较HSP组低(P<0.05);CSP组术后应激指标水平小于HSP组(P<0.05)。结论:与内镜HSP相比,内镜CSP治疗直径≤10mmCP患者的效果相当,但操作时间短,手术安全性高,治疗花费少,同时在手术过程中对患者的应激反应更轻,可为患者的术后恢复创造有利条件。Objective:To compare and analyze the efficacy of endoscopic snare cold resection(CSP)and endoscopic snare thermal resection(HSP)in the treatment of colorectal polyps(CP)with diameter≤10mm.Methods:180 patients with diameter≤10mmCP in our hospital from October 2021 to January 2024 were collected and divided into CSP group(90 cases)and HSP group(90 cases)according to different surgical methods.The therapeutic effects of the two groups were compared.Results:The time of polypectomy,the number of titanium clips used and the cost of treatment in CSP group were lower than those in HSP group(P<0.05),and there were no significant differences in the complete polypectomy rate,complete polypectomy rate and polyp recovery rate among groups(P>0.05).The incidence of intraoperative bleeding in CSP group was higher than that in HSP group,and the incidence of delayed bleeding,submucosal involvement injury,perforation and electrocoagulation syndrome was lower than that in HSP group(P<0.05).The level of postoperative stress index in CSP group was lower than that in HSP group(P<0.05).Conclusion:Compared with endoscopic HSP,endoscopic CSP has the same effect in the treatment of patients with diameter≤10mmCP,but the operation time is short,the operation safety is high,the treatment cost is low,and the stress reaction to patients is lighter during the operation,which can create favorable conditions for the postoperative recovery of patients.
