摘 要:目的:分析调强适形放射治疗(IMRT)对乳腺癌根治术后患者的影响。方法:回顾性分析2020年1月~2023年10月莆田市第一医院收治的90例乳腺癌根治术后患者的临床资料,按治疗方式不同将其分为两组。行三维适形放射治疗(3D-CRT)的45例患者纳入对照组,行IMRT的45例患者纳入观察组。对比两组肿瘤标志物水平、剂量学参数、不良反应。结果:治疗前后,两组各项肿瘤标志物水平相比无明显差异(P>0.05);两组V95%、适形指数相比无明显差异(P>0.05);观察组V105%、V110%、均匀性指数、患侧心脏与肺,对侧乳腺、肝脏受照剂量均低于对照组;不良反应发生率为17.78%(8/45),低于对照组的42.22%(19/45),差异有统计学意义(P<0.05)。结论:IMRT与3D-CRT均能抑制乳腺癌根治术后患者的癌细胞增殖,但IMRT更能降低靶区与器官的照射剂量,且不良反应更少,存在一定的应用价值。Objective:To analyze the effect of intensity modulated radiotherapy(IMRT)on breast cancer patients after radical mastectomy.Methods:The clinical data of 90 patients with breast cancer after radical mastectomy admitted to Putian First Hospital from January 2020 to October 2023 were retrospectively analyzed,and they were divided into two groups according to different treatment methods.45 patients who underwent three-dimensional conformal radiotherapy(3D-CRT)were included in the control group,and 45 patients who underwent IMRT were included in the observation group.Compare the levels of tumor markers,dosimetric parameters,and adverse reactions between two groups.Results:There was no significant difference in the levels of various tumor markers between the two groups before and after treatment(P>0.05);There was no significant difference in V95%and CI between the two groups(P>0.05);The observation group received lower doses of V105%,V110%,uniformity index(HI),affected heart and lung,contralateral breast,and liver compared to the control group;The incidence of adverse reactions was 17.78%(8/45),lower than the control group’s 42.22%(19/45)(P<0.05).Conclusion:Both IMRT and 3D-CRT can inhibit the proliferation of cancer cells in breast cancer patients after radical mastectomy,but IMRT can reduce the radiation dose of target areas and organs more,with fewer adverse reactions,which has certain application value.
