睿米机器人辅助经皮微球囊压迫术治疗原发三叉神经痛的疗效

摘　　要：目的探讨睿米机器人辅助经皮微球囊压迫术(PBC)治疗原发性三叉神经痛(PTN)的疗效。方法回顾性分析2021年1月至2022年6月使用睿米机器人辅助PBC治疗的30例PTN的临床资料。结果30例均注册成功并顺利完成手术,一次穿刺成功16例,二次调整14例;术中DSA侧位像显示微球囊在Meckel腔呈“梨形”。术后即刻疼痛均明显缓解,VAS评分较术前明显降低(P<0.05);随访6个月,疼痛无复发。术后即刻4例(13.3%)出现咀嚼肌无力,2例(6.7%)出现同侧感觉异常。术后即刻,所有病人均出现面部麻木,其中轻度15例,中重度15例;术后6个月,面部麻木程度部分缓解,其中轻度麻木22例,中重度8例。结论睿米机器人辅助PBC治疗PTN具有定位精准、安全的优点,是一种可行的、有效的手术方法。Objective To investigate the clinical efficacy of Ruimi robot-assisted percutaneous microballoon compression(PBC)for patients with primary trigeminal neuralgia(PTN).Methods A retrospective analysis was conducted on the clinical data of 30 patients with PTN who underwent Ruimi robot-assisted PBC from January 2021 to June 2022.Results All 30 patients successfully registered and completed the operation.Among them,16 patients had a successful single puncture,and 14 patients required adjustments.The intraoperative lateral DSA images revealed that the microballoon in the Meckel's cavity presented a typical"pear-shaped"form.Immediately after the operation,the pain was significantly alleviated in all patients,and the VAS score was significantly lower than that before the operation(P<0.05);during the 6-month follow-up,there was no recurrence of pain.Immediately after the operation,4 cases(13.3%)presented with masticatory muscle weakness,and 2 cases(6.7%)had ipsilateral sensory abnormalities.Immediately after the operation,all patients had facial numbness,among which 15 cases were mild,and 15 cases were moderate to severe;at 6 months after the operation,the degree of facial numbness was partially relieved,among which 22 cases were mild,and 8 cases were moderate to severe.Conclusion Ruimi robot-assisted PBC in the treatment of PTN has the advantages of precise positioning and safety,and is a feasible and effective surgical method.
