摘　　要：目的探讨应用Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤的有效性及安全性。方法回顾性分析2022年3月至2023年2月应用Neuroform Atlas支架辅助弹簧圈栓塞治疗的29例颅内宽颈动脉瘤的临床资料。结果29例均为单发动脉瘤,其中9例为破裂动脉瘤,20例为未破裂动脉瘤。术中共置入29枚Neuroform Atlas支架,均成功释放,术中发生操作相关并发症1例。术后即刻DSA显示,改良Raymond分级Ⅰ级25例(86.2%),Ⅱ级1例(3.4%),Ⅲa级3例(10.3%)。术后1例因急性肺栓塞死亡。术后随访3~6个月,28例完成临床随访,mRS评分0分26例,1分1例,3分1例;18例完成影像学随访,改良Raymond分级Ⅰ级15例(83.3%),Ⅱ级1例(5.6%),Ⅲa级例2(11.1%),未发现载瘤动脉狭窄或迟发性血栓事件。结论Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤具有较高的动脉瘤闭塞率和较低的围手术期并发症发生率,安全性和有效性均较好,但其远期疗效仍需进一步随访观察。Objective To investigate the safety and efficacy of Neuroform Atlas stent-assisted coiling in the treatment of intracranial wide-necked aneurysms.Methods This was a retrospective analysis of 29 patients harboring 29 intracranial wide-necked aneurysms treated with Neuroform Atlas stent-assisted coiling from March 2022 to February 2023.Results Of the 29 aneurysms,9 patients presented with acute ruptured aneurysms and 20 patients with unruptured aneurysms.A total of 29 Neuroform Atlas stents were successfully releases in 29 cases and there was 1 case with intraoperative complication.Immediate postoperative DSA revealed that 25 cases(86.2%)were classified as gradeⅠof the modified Raymond scale,1 case(3.4%)was classified as gradeⅡ,and 3 cases(10.3%)were classified as gradeⅢa.After the operation,1 patient died of acute pulmonary embolism.During the postoperative follow-up period of 3~6 months,the clinical follow-up was completed in 28 cases,of whom 26 cases had an mRS score of 0,1 case had a score of 1,and 1 case had a score of 3;the imaging follow-up was completed in 18 cases,of whom 15 cases(83.3%)were classified as gradeⅠof the modified Raymond scale,1 case(5.6%)was classified as gradeⅡ,and 2 cases(11.1%)were classified as gradeⅢa.No stenosis of the parent artery or delayed thrombotic events were found.Conclusion Neuroform Atlas stent-assisted coil embolization for wide-necked intracranial aneurysms has a high aneurysm occlusion rate and a low incidence of perioperative complications,with good safety and efficacy.However,its long-term efficacy still requires further follow-up and observation.
