晚期三阴乳腺癌经消肿散结汤辅助多西他赛联合卡培他滨化疗方案治疗的效果观察

摘　　要：目的:分析消肿散结汤辅助多西他赛联合卡培他滨(TX)化疗方案治疗晚期三阴乳腺癌(Triple negative breast cancer,TNBC)的疗效。方法:回顾性分析2021年1月—2024年1月河南省南阳市第一人民医院86例晚期TNBC患者资料,按不同治疗方案分为对照组和观察组各43例。对照组TX化疗方案治疗,观察组消肿散结汤+TX化疗方案治疗。比较2组临床疗效、中医证候积分、肿瘤标志物[癌胚抗原(CEA)、糖类抗原(CA125、CA153)]、生活状态评价卡氏(KPS)评分及毒副反应。结果:观察组临床客观缓解率58.14%(25/43)高于对照组34.88%(15/43)(P<0.05);治疗后观察组中医证候积分(头晕目眩、乳房肿块、消瘦纳呆、气短乏力)低于对照组(P<0.05);治疗后观察组CEA、CA125、CA153低于对照组,KPS评分高于对照组(P<0.05);2组白细胞减少、恶心呕吐、肝肾损伤、脱发、血小板减少发生率比较,无明显差异(P>0.05)。结论:消肿散结汤联合TX化疗方案治疗晚期TNBC疗效显著,可减轻患者症状,降低肿瘤标志物表达,改善患者体能状态,且安全性较好。Objective:To analyze the efficacy of Xiaozhong Sanjie Decoction assisted by docetaxel combined with capecitabine(TX)chemotherapy in the treatment of advanced triple-negative breast cancer(TNBC).Methods:A retrospective analysis was performed on 86 patients with advanced TNBC admitted to the First People's Hospital of Nanyang City,Henan Province from January 2021 to January 2024.The patients were divided into a control group and an observation group,with 43 cas es in each group,according to different treatm ent regimens.The control group was treated with TX chemotherapy,and the observation group was treated with Xiaozhong Sanjie Decoction+TX chemotherapy.The clinical efficacy,TCM syndrome scores,tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigens(CA125,CA153)],Karnofsky performance status(KPS)score,and toxic and side effects were compared between the two groups.Results:The clinical objective remission rate of the observation group was 58.14%(25/43),which was higher than that of the control group 34.88%(15/43)(P<0.05).After treatment,the TCM syndrome score(dizziness,breast mass,weight loss,poor appetite,shortness of breath and fatigue)of the observation group was lower than that of the control group(P<0.05).After treatment,the CEA,CA125,and CA153 of the observation group were lower than those of the control group,and the KPS score was higher than that of the control group(P<0.05).There was no significant difference in the incidence of leukopenia,nausea and vomiting,liver and kidney damage,alopecia,and thrombocytopenia between the two groups(P>0.05).Conclusion:Xiaozhong Sanjie Decoction combined with TX chemotherapy regimen has significant efficacy in the treatment of advanced TNBC,can alleviate the symptoms of patients,reduce the expression of tumor markers,improve the physical condition of patien ts,and has good safety.
