良性阵发性位置性眩晕患者复位前后脑网络功能连通性分析

摘　　要：目的观察良性阵发性位置性眩晕(BPPV)患者耳石复位法治疗前后脑网络功能连通性(FC)的变化,分析BPPV患者FC与眩晕程度(THI)之间的相关性,进而从形态学角度探寻BPPV的中枢机制。方法用功能性近红外线光谱成像技术(fNIRS)测试BPPV患者复位治疗前后FC的变化,将BPPV患者FC与DHI量表评分进行相关性分析。结果BPPV组复位治疗前后FC较正常对照组低;BPPV组DHI评分与FC负相关(P<0.01)。BPPV患者复位治疗后视觉皮层V2+V3、视觉皮层V1、体感皮层的FC较治疗前升高(P<0.01)。结论与正常对照组相比,BPPV患者全脑FC下降,主要累及到前额叶皮层、视觉皮层、体感皮层;复位治疗后BPPV患者FC增加,但未达到正常水平,这可能与BPPV患者复位治疗后持续存在头晕有一定的关联。BPPV患者脑网络FC越低,眩晕程度越重。Objective To observe changes of brain functional connectivity(FC)in patients with benign paroxysmal positional vertigo(BPPV)before and after canalith repositioning procedure(CRP),and to analyze the correlation between FC and vertigo degree(THI)in patients with BPPV,so as to explore the central mechanism of BPPV from the perspective of morphology.Methods Functional near-infrared imaging(fNIRS)was used to test the changes of FC in patients with BPPV before and after CRP treatment,and the correlation between FC and DHI scale scores in patients with BPPV was analyzed.Results The FC of BPPV group was lower than that of normal control group before and after CRP treatment.In BPPV group,DHI score was negatively correlated with FC(P<0.01).After CRP treatment,the FC of visual cortex V2+V3,visual cortex V1 and somatosensory cortex in BPPV patients were higher than those before treatment(P<0.01).Conclusion Compared with normal control group,the whole brain FC of patients with BPPV decreases,mainly involving the prefrontal cortex,visual cortex and somatosensory cortex.After CRP treatment,the FC of BPPV patients increases,but does not reach the normal level,which may be related to the persistent dizziness after CRP treatment in BPPV patients.The lower the FC of the brain network in BPPV patients,the more severe the vertigo.
