塞内加尔医疗卫生机构服务准备度对发热儿童卫生服务质量的影响

摘　　要：目的:了解塞内加尔医疗卫生机构为发热儿童提供卫生服务的准备度现状,并分析服务准备度对卫生服务质量的影响,为塞内加尔及与其相似的医疗资源匮乏地区优化卫生资源配置、促进服务质量提升提供参考。方法:基于塞内加尔2013-2019年医疗卫生机构服务提供调查数据进行分析。根据世界卫生组织服务可用性和准备度评估指南,从数据库中提取指标,计算医疗卫生机构准备度得分;根据世界卫生组织儿童疾病综合管理指南计算发热儿童卫生服务质量得分。运用多因素线性回归模型分析机构准备度对服务质量的影响。结果:共纳入1919家医疗卫生机构,平均服务准备度得分为0.67(满分为1)。来自上述机构的共2013名服务提供者为发热儿童提供服务,平均服务质量得分为0.36(满分为1)。机构服务准备度与服务质量呈显著的正相关关系(β=0.071,P=0.026)。结论:塞内加尔医疗卫生机构并未做好为发热儿童提供卫生服务的充分准备,这在一定程度上会影响卫生工作者为患儿提供的服务质量。因此,该国政府需要注重提升医疗卫生机构的服务准备度,从而促进卫生服务质量不断优化。Objective:To evaluate the service readiness of health facilities for febrile children in Senegal and analyze the influence of facility readiness on health service quality,so as to provide reference for optimizing the allocation of health resources and improving the quality of health services in Senegal and other similar areas with limited medical resources.Methods:Data used in this study came from the Senegal Service Provision Assessment(SPA)surveys conducted from 2013 to 2019.According to the Service Availability and Readiness Assessment(SARA)manual,indicators were extracted from the database to construct the facility readiness score.The quality score was based on the Integrated Management of Childhood Illnesses(IMCI).Multivariate linear regression model was used to analyze the influence of facility readiness on service quality.Results:A total of 1919 health facilities were included with an average service readiness score of 0.67(out of 1).2013 health providers from these facilities provided service for febrile children with an average quality score of 0.36(out of 1).It was found that health facility readiness have a positive impact on service quality(β=0.071,P=0.026).Conclusion:Health facilities in Senegal are not fully ready to provide health service for febrile children,which affect the quality of service provided by health providers.Therefore,the government should focus on improving the service readiness of health facilities in order to promote the quality of health services.
