摘 要:目的 分析血清蛋白酶体及胸苷激酶1水平在预测食管鳞癌术后放化疗患者复发中的应用价值,为食管鳞癌的二级和三级预防提供参考依据。方法 选择2018年1月至2020年2月郑州市第三人民医院收治的64例食管鳞癌患者为研究对象,所有患者均接受手术及术后放化疗干预方案,根据是否复发分为复发组与未复发组,每组32例。比较两组血清蛋白酶体及胸苷激酶1水平,分析其在预测食管鳞癌术后放化疗患者复发中的价值。结果 治疗前,两组血清蛋白酶体及胸苷激酶1水平比较,差异未见统计学意义(P>0.05)。治疗后,未复发组血清蛋白酶体及胸苷激酶1水平均明显低于复发组,差异有统计学意义(P<0.001)。食管癌胸腔镜治疗预后不良与患者血清蛋白酶体及胸苷激酶1水平密切相关,且两项联合检测的灵敏度、特异度均高于单项检测,差异有统计学意义(P<0.05)。结论 食管鳞癌术后放化疗复发患者血清蛋白酶体及胸苷激酶1水平表达上升,联合检测在诊断食管鳞癌术后放化疗复发中价值较高。Objective e To analyze the value of serum proteasome and thymidine kinase 1 levels in predicting the recurrence of esophageal squamous cell carcinoma after postoperative radiotherapy and chemotherapy,and to provide reference for the secondary and tertiary prevention of esophageal squamous cell carcinoma.Methods s Sixty-four patients with esophageal squamous cell carcinoma admitted to the Third People's Hospital of Zhengzhou from January 2018 to February 2020 were selected as the study objects.All patients received surgery and postoperative chemoradiotherapy intervention programs,and were divided into a relapsed group and a non-relapsed group,with 32 cases in each group.The serum proteasome and thymidine kinase 1 levels were compared between the two groups to analyze their value in predicting the recurence of esophageal squamous cell carcinoma after radiotherapy and chemotherapy.Results Before treatment,there was no significant difference in serum proteasome and thymidine kinase 1 levels between the two groups(P>0.05).After treatment,the levels of serum proteasome and thymidine kinase 1 in the non-relapsed group were significantly lower than those in the relapsed group,with significant differences(P<0.001).The adverse prognosis of thoracoscopic treatment of esophageal cancer was closely related to the levels of serum proteasome and thymidine kinase 1,and the sensitivity and specificity of the two combined tests were higher than that of the single test,with significant differences(P<0.05).Conclusions The levels of serum proteasome and thymidine kinase 1 increased in patients with esophageal squamous cell carcinoma recurrence after radiotherapy and chemotherapy,and the combined detection is of high value in the diagnosis of esophageal squamous cell carcinoma recurrence after radiotherapy and chemotherapy.
