消化性溃疡患者停药后HP复阳的相关影响因素分析

摘　　要：目的 探讨消化性溃疡患者停药后幽门螺杆菌(HP)复阳的影响因素.方法 选取2021年1月-2022年12月鹰潭市人民医院收治的消化性溃疡患者81例,所有患者均接受治疗,并经HP检查为阴性后停药,同时停药4周后再次检查消化性溃疡患者HP复阳情况,根据结果分为复阳组与未复阳组,详细统计两组患者的基线资料并比较,重点分析消化性溃疡患者HP复阳的影响因素.结果 经评估,81例消化性溃疡患者停药后HP复阳40例,占比49.38%(40/81);复阳组与未复阳组患者的吸烟史、口腔HP感染、焦虑及不良生活习惯史比较,有统计学差异(P<0.05),组间性别、年龄、溃疡类型、饮酒史、家庭月人均收入比较,无统计学差异(P>0.05);经回归分析,结果显示,吸烟史、口腔HP感染、焦虑及不良生活习惯史均是消化性溃疡患者停药后HP复阳的影响因素(OR>1,P<0.05).结论 消化性溃疡患者停药后HP复阳风险较高,吸烟史、口腔HP感染、焦虑及不良生活习惯史均是消化性溃疡患者停药后HP复阳的影响因素.Objective To investigate the influence factors of Helicobacter pylori(HP)relapse in patients with peptic ulcer after discontinuation of medication.Methods A total of 81 patients with peptic ulcer admitted to the hospital from January 2021 to December 2022 were selected,all of whom received treatment and stopped taking HP after negative test.At the same time,the HP relapse of peptic ulcer patients was checked again 4 weeks later,and the patients were divided into the relapse group and the non-relapse group according to the results,and the baseline data of the two groups were analyzed in detail and compared.The influence factors of HP reactivation in patients with peptic ulcer were analyzed.Results In the 81 patients with peptic ulcer,40 cases[49.38%(40/81)]recovered HP after withdrawal.There were statistical differences in smoking history,oral HP infection,anxiety and bad lifestyle history between the two groups(P<0.05),while there were no statistical differences in gender,age,ulcer type,drinking history and monthly family per capita income between the two groups(P>0.05).By regression analysis,the results showed that smoking history,oral HP infection,anxiety and bad lifestyle history were all the influencing factors of HP recovery after withdrawal of peptic ulcer patients(OR>1,P<0.05).Conclusion The risk of HP relapse is higher in patients with peptic ulcer after drug withdrawal.Smoking history,oral HP infection,anxiety and bad lifestyle history are all factors affecting HP relapse after drug withdrawal.
