综合康复治疗DPN患者的疗效及对周围神经功能的影响

摘　　要：目的探究综合康复治疗糖尿病周围神经病变(DPN)患者的疗效及对周围神经功能的影响.方法选择2022年3月-2024年2月在吉林大学第三临床医院收治的91例DPN患者为研究对象,依据随机数字表法将其分为观察组(n=46)和对照组(n=45).对照组予以常规治疗,观察组在对照组基础上予以综合康复治疗,两组均治疗8周.对比两组临床疗效、血液流变学水平、临床证候评分、神经传导速度.结果观察组总有效率为97.83%,高于对照组的86.67%,差异有统计学意义(P<0.05);治疗后,观察组全血高切黏度(HSV)、全血低切黏度(LSV)、血浆黏度(PSV)、血细胞比容(HCT)低于对照组(P<0.05);治疗后,观察组临床证候各项评分低于对照组(P<0.05);治疗后,观察组正中神经与腓总神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)高于对照组(P<0.05).结论综合康复治疗在DPN中疗效显著,能减轻患者临床症状,改善其血液流变学水平,并恢复其周围神经传导速度.Objective To explore the effect of comprehensive rehabilitation on diabetic peripheral neuropathy(DPN)patients and its influence on peripheral nerve function.Methods A total of 91 DPN patients admitted to the Third Clinical Hospital of Jilin University from March 2022 to February 2024 were selected as the study objects and divided into observation group(n=46)and control group(n=45)according to random number table method.The control group was given routine treatment,and the observation group was given comprehensive rehabilitation treatment on the basis of the control group.Both groups were treated for 8 weeks.The clinical efficacy,hemorheology level,clinical symptom score and nerve conduction velocity were compared between the two groups.Results The total effective rate of the observation group was 97.83%,higher than that of the control group(86.67%),the difference was statistically significant(P<0.05).After treatment,whole blood high tangential viscosity(HSV),whole blood low tangential viscosity(LSV),plasma viscosity(PSV)and hemicytosis(HCT)in observation group were lower than those in control group(P<0.05).After treatment,the scores of clinical symptoms in the observation group were lower than those in the control group(P<0.05);after treatment,the motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of median nerve and common peroneal nerve in observation group were higher than those in control group(P<0.05).Conclusion Comprehensive rehabilitation therapy is effective in DPN,which can reduce the clinical symptoms,improve the level of hemorheology and restore the peripheral nerve conduction velocity.
