甲状腺乳头状癌中CK19、HBME1、Galectin-3和COX-2的表达及其与临床病理学特征的相关性

摘  要:目的 探讨甲状腺乳头状癌(Papillary Thyroid Carcinoma,PTC)中CK19、HBME1、Galectin-3和COX-2的表达及其与临床病理学特征的相关性。方法 回顾性分析2021年1月至2023年12月本院收治的PTC患者临床资料中,手术切除的143例甲状腺乳头状癌病理组织标本,免疫组化染色,观察CK19、HBME1、Galectin-3和COX2的表达与患者肿瘤分期、淋巴结转移、甲状腺外浸润的临床病理学特征的相关性。结果 TNM分期T3和T4期、T2期以及有淋巴结转移、有甲状腺外浸润的患者,CK19、HBME1、Galectin-3和COX-2四种标志物的表达阳性率显著高于TNM分期T1期、无淋巴结转移、无腺外浸润的患者(P<0.05)。此外,CK19、HBME1、Galectin-3和COX-2在PTC组织中的共阳性表达率为70.63%,呈正相关(r=5.684,P<0.001)。结论 CK19、HBME1、Galectin-3和COX-2的表达与PTC患者的肿瘤分期、淋巴结转移、甲状腺外浸润密切相关,4者表达共阳性可能提示PTC患者的肿瘤分期较高,癌侵袭性较强,且发生淋巴结转移的可能性增加。Objective To explore the expression of CK19,HBME1,Galectin-3 and COX-2 in papillary thyroid carcinoma(PTC) and their correlation with clinicopathological features.Methods A retrospective analysis was conducted on the clinical data of 143 PTC patients who underwent surgical resection of thyroid papillary carcinoma tissue at our hospital from January 2021 to December 2023.Immumohistochemical staining was performed to observe the correlation between the expression of CK19,HBME1,Galectin-3 and COX-2 and the clinicopathological features of tumor stage,lymph node metastasis and extrathyroidal extension.Results The positive expression rates of CK19,HBME1,Galectin-3 and COX-2 were significantly higher in patients with TNM stages T3 and T4,T2,lymph node metastasis,and extrathyroidal extension than in those with TNM stage T1,without lymph node metastasis,and without extrathyroidal extension(P<0.05).In addition,the co-positive expression rates of CK19,HBME1,Galectin-3 and COX-2 in PTC tissues were 70.63%,indicating a positive correlation(r=5.684,P<0.001).Conclusion The expressions of CK19,HBME1,Galectin-3,and COX-2 are closely related to tumor stage,lymph node metastasis,and extrathyroidal extension in PTC patients,and the co-positive expression of the four indicators suggest a higher tumor stage,stronger cancer invasiveness,and an increased likelihood of lymph node metastasis in PTC patients.
