摘 要:目的探讨通过神经突方向离散度与密度成像(NODDI)观察不同体积下的假定血管源性高信号(WMH)表现并探究该技术的应用价值。方法选取106例诊断为假定血管源性WMH患者的影像资料,比较正常白质(NAWM)和WMH的NODDI与伪彩图(DTI)参数间的差异以及相关性。比较不同WMH体积分组间的NAWM与WMH的各参数的差异。偏相关分析NODDI、DTI参数与WMH体积的相关性。采用多元线性回归分析WMH体积增大的相关因素。结果1)WMH区域的FA、NDI、ODI值低于NAWM区域,MD、Viso值高于NAWM区域,差异有统计学意义(P<0.05);2)WMH的FA与ODI值以及NAWM的Viso和MD值在四个分组差异无统计学意义(P>0.05),WMH的MD、NDI、Viso值以及NAWM的FA、NDI、ODI值差异有统计学意义(P<0.05);3)WMH的FA、MD分别与WMH的ODI和Viso的相关系数差异有统计学意义(P<0.001),NAWM的FA、MD分别与相应的NODDI参数的相关系数差异有统计学意义(P<0.001);4)WMH的MD、NDI、ODI、Viso值,NAWM的FA、NDI、ODI值与WMH体积的相关系数差异有统计学意义(P<0.05);5)线性回归分析显示WMH-Viso、WMH-NDI、NAWM-ODI分别和WMH体积独立相关(P<0.05)。结论相比于FA、MD之类的DTI指标,NODDI参数对WMH的敏感性更高,也更能特异性地反映正常白质的变化差异。Objective To observe the manifestations of presumed vascular white matter hyperintensity(WMH)in different volumes by neurite orientation dispersion and density imaging(NODDI)and to explore the application value of this technique.Methods The imaging data of 106 patients with putative vascular WMH were selected.The differences and correlations between NODDI and DTI parameters in NAWM and WMH were compared.The differences of NAWM and WMH parameters between different WMH volume groups were compared.Partial correlation analysis was used to analyze the correlation between NODDI,DTI parameters and WMH volume.Multiple linear regression was used to analyze the related factors of WMH volume enlargement.Results 1)The FA,NDI and ODI values of WMH were significantly lower than those of NAWM,and the MD and Viso values were significantly higher than those of NAWM(P<0.05);2)There were no significant differences in FA and ODI values of WMH and Viso and MD values of NAWM among the four groups(P>0.05).MD,NDI and Viso values of WMH and FA,NDI and ODI values of NAWM were significantly different among the four groups(P<0.05);3)The FA and MD of WMH were significantly correlated with ODI and Viso of WMH(P<0.001),and the FA and MD of NAWM were significantly correlated with parameters of NODDI(P<0.001);4)The MD,NDI,ODI,Viso of WMH,FA,NDI,ODI of NAWM were significantly correlated with the volume of WMH(P<0.05);and 5)Linear regression analysis showed that WMH-Viso,WMH-NDI,and NAWM-ODI were independently correlated with the volume of WMH(P<0.05).Conclusion NODDI parameters are more sensitive to WMH than other DTI parameters such as FA and MD,which can more specifically reflect the changes of normal white matter.
