摘 要:目的探讨不同程度炎症性肠病(IBD)患儿微剂量CT全肠道造影(CTE)表现及其定量参数改变情况。方法选取本院117例IBD患儿资料,根据病情程度分为轻度组26例、中度组68例、重度组23例,均行微剂量CTE比较、分析3组CTE表现及定量参数(CT值、碘含量)与IBD病情程度的关系及评估价值。结果重度组肠壁增厚、肠壁分层、黏膜下气泡、肠系膜淋巴结肿大、结肠袋消失占比>中度组>轻度组,重度组CTE影像表现评分>中度组>轻度组,差异有统计学意义(P<0.05);重度组动脉期、门静脉期40~100 keV的CT值、碘浓度>中度组>轻度组,差异有统计学意义(P<0.05);肠壁增厚、肠壁分层、黏膜下气泡、肠系膜淋巴结肿大、结肠袋消失、影像表现评分与IBD病情程度呈正相关(P<0.05);动脉期、门静脉期40~100 keV的CT值、碘浓度与IBD病情程度呈正相关(P<0.05);CTE影像表现评分、动脉期、门静脉期40~100 keV的CT值、碘浓度鉴别轻度与中度IBD、评估重度IBD的AUC均在0.7以上。结论IBD患儿微剂量CTE影像学表现、定量参数与病情程度密切相关,其对IBD患儿病情程度具有良好的评估价值。Objective To investigate the performance of low-dose CT enterography(CTE)and the changes in quantitative parameters in children with different degrees of inflammatory bowel disease(IBD).Methods A total of 117 children with IBD were selected from our hospital,and were divided into mild group(26 cases),moderate group(68 cases),and severe group(23 cases)according to the severity of their disease.All of them underwent microdose CTE,and the relationship between the CTE manifestations and quantitative parameters(CT value,iodine content)and the severity of IBD,as well as their evaluation value,were compared and analyzed..Results The proportions of thickening of the intestinal wall,layering of the intestinal wall,submucosal bubbles,enlargement of mesenteric lymph nodes,and disappearance of colonic pouches in the three groups were compared as follows:severe group>moderate group>mild group.The CTE imaging performance scores in the three groups were compared as follows:severe group>moderate group>mild group(P<0.05);the CT values and iodine concentrations in the arterial and portal venous phases at 40-100 keV in the three groups were compared as follows:severe group>moderate group>mild group(P<0.05);the thickness of the intestinal wall,layering of the intestinal wall,submucosal bubbles,enlargement of mesenteric lymph nodes,disappearance of colonic pouches,and imaging performance scores were positively correlated with the severity of IBD(P<0.05);the CT values and iodine concentrations in the arterial and portal venous phases at 40-100 keV were positively correlated with the severity of IBD(P<0.05);the AUCs of CTE imaging performance scores,CT values in the arterial and portal venous phases at 40-100 keV,and iodine concentrations for identifying mild and moderate IBD and evaluating severe IBD were all above 0.7.Conclusion The imaging findings and quantitative parameters of low-dose CTE in children with IBD are closely related to the severity of the disease,which has good evaluation value for the severity of IBD in children.
