温阳针灸法联合通络扶正汤治疗恢复期气虚血瘀型脑梗死的效果及对血清Toll样受体、白细胞介素-6的影响

摘　　要：目的 探讨温阳针灸法联合通络扶正汤治疗恢复期气虚血瘀型脑梗死的效果及对血清Toll样受体(TLR)、白细胞介素-6(IL-6)的影响。方法 选取浙江大学医学院附属第二医院临平院区收治的210例气虚血瘀型恢复期脑梗死患者,按照随机数字表法将其分为对照组1(通络扶正汤治疗)、对照组2(温阳针灸法治疗)与联合组(温阳针灸法联合通络扶正汤治疗),各70例。三组均治疗8周。比较三组治疗前后中医症候积分、脑血流动力学和血清TLR2、TLR3、IL-6水平,评估临床疗效,记录不良反应。结果 治疗后,三组各症候积分、搏动指数、阻力指数及血清TLR2、IL-6水平较治疗前降低,且联合组低于对照组1、对照组2(P<0.05)。治疗后,三组平均血流速度及血清TLR3水平较治疗前升高,且联合组高于对照组1、对照组2(P<0.05)。联合组临床疗效优于对照组1、对照组2(P<0.05)。三组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 温阳针灸法联合通络扶正汤治疗恢复期气虚血瘀型脑梗死具有良好效果。Objective To investigate the efficacy and effect on serum Toll-like receptor(TLR)and interleukin-6(IL-6)of acupuncture and moxibustion for warming yang combined with Tongluo Fuzheng Decoction in the treatment of cerebral infarction with qi deficiency and blood stasis in the recovery period.Methods A total of 210 patients with recovering cerebral infarction of the qi deficiency and blood stasis type admitted to the Linping Branch,Second Affiliated Hospital,School of Medicine,Zhejiang University were selected from March 2022 to March 2024,and they were divided into the control group 1(treated with Tongluo Fuzheng Decoction),control group 2(treated with acupuncture and moxibustion for warming yang),and the combined group(acupuncture and moxibustion for warming yang combined with Tongluo Fuzheng Decoction)by random number table method,with 70 cases in each group,and all three groups were treated for 8 weeks.The traditional Chinese medicine symptom scores,cerebral hemodynamics,and serum TLR2,TLR3,and IL-6 levels were compared among the three groups before and after treatment,clinical efficacy was assessed and adverse effects were recorded.Results After treatment,the symptom scores,beat-to-beat index,resistance index,and serum TLR2 and IL-6 levels of the three groups were lower than those before treatment,and the combined group was lower than control group 1 and control group 2(P<0.05).After treatment,the mean blood flow velocity and serum TLR3 levels of the three groups were higher than before treatment,and the combined group was higher than control group 1 and control group 2(P<0.05).The clinical efficacy of the combined group was better than that of control group 1 and control group 2(P<0.05).There were no statistically significant difference in the incidence of total adverse reactions among the three groups(P>0.05).Conclusion Acupuncture and moxibustion for warming yang combined with Tongluo Fuzheng Decoction has a good effect in the treatment of cerebral infarction of qi deficiency and blood stasis type in the rec
