肛周巨大表皮样囊肿2例并文献复习

摘　　要：直径>5 cm的肛周巨大表皮样囊肿临床中比较罕见,处理时更为棘手,治疗不及时或囊肿自行破溃,易误诊为肛周脓肿或肛瘘等疾病,若切除不彻底或单纯的切开引流,可引起较高的复发率,为临床医务工作者提出一定的难题与挑战。本病一般不与肛门直肠相通,但随着肿物逐渐增大,尚存在破裂或继发感染的风险。无论有无临床症状,均建议尽早手术,手术彻底切除囊肿与囊壁组织是治疗本病的重要原则,并强调肿物切除的完整性以降低术后复发率。本文回顾分析了2例肛周巨大表皮样囊肿的诊治经过,并结合文献复习,以期为临床诊疗提供参考。Giant perianal epidermoid cysts with a diameter greater than 5 cm are relatively rare in the clinic,and are more dificult to deal with.Delayed treatment or spontaneous rupture of cysts can easily be misdiagnosed as other diseases such as perianal abscess or anal fistula.If the resection is incomplete or simple incision and drainage are performed,it can cause a high recurrence rate,which poses a certain challenges and difficulties for clinical medical workers.This disease is generally not connected to the anus and rectum,but with the gradual enlargement of the mass,there are still risks of rupture and secondary infection.Regardless of whether there are clinical symptoms,it is recommended to surgery as soon as possible.Surgical completely excision of the cyst and parietal tissue is an important principle of treatment,and the integrity of the mass resection is emphasized to reduce the postoperative recurrence rate.This article reviews the diagnosis and treatment of two cases of giant perianal epidermoid cysts and combines literature review in order to provide reference for clinical diagnosis and treatment.
