摘 要:创伤性颅脑损伤(TBI)是全球性的公共卫生挑战,具有高发病率、高致死率和高致残率。近年来,针灸因其独特疗效在TBI的临床治疗中被广泛应用,但对其作用机制的探讨相对较少。本文通过梳理近5年针灸治疗TBI的相关研究发现,针灸可通过促进神经修复与再生、抗炎及调节细胞凋亡、细胞自噬、突触可塑性和细胞能量代谢等多靶点改善TBI的临床症状。但仍缺乏系统深入高质量的研究,未来研究应围绕血脑屏障保护、脑肠轴等热点研究更深入探索针灸起效机制,为阐明针灸治疗TBI的潜在作用提供科学依据。Traumatic brain injury(TBI)is a global public health challenge with a high incidence rate,mortality rate,and disability rate.In recent years,acupuncture and moxibustion has been widely used in clinical treatment of TBI due to its unique efficacy,but there is relatively little discussion on its mechanism of action.This article reviews relevant research findings of acupuncture and moxibustion in the treatment of TBI in the past five years,and finds that acupuncture and moxibustion can improve clinical symptoms of TBI through multiple targets such as promoting neural repair and regeneration,anti-inflammation,regulating cell apoptosis,cell autophagy,synaptic plasticity,and cellular energy metabolism.However,there is still a lack of systematic and in-depth high-quality research.Future research should focus on hot topics such as blood-brain barrier protection and brain-gut axis to explore the mechanism of acupuncture and moxibustion,providing scientific evidence for clarifying the potential role of acupuncture in the treatment of TBI.
