国家公立医院绩效考核新模式下运用疾病诊断相关分组对某三甲中医院康复科住院绩效评价分析

摘　　要：目的探讨在国家公立医院绩效考核新模式下,运用疾病诊断相关分组(DRG)对三甲中医院康复科进行住院绩效评价的效果分析。方法选取广东省第二中医院康复科7个病区2022年1月至12月实施DRG评价前(对照组)及2023年1月至12月实施DRG评价后(观察组)的住院病案首页信息,以国家版CN-DRGs分组,共入组10527例,其中对照组4947例,观察组5580例。比较分析实施DRG评价前后DRG指标及国家公立医院绩效考核住院中医特色指标数据的变化及差异。结果观察组总权重、病例组合指数、疑难重症例数高于对照组(P<0.05);观察组时间消耗指数、费用消耗指数低于对照组(P<0.05);观察组DRG综合能力得分高于对照组(P<0.05);观察组以中医为主治疗的出院患者比例、中药饮片收入占药品收入比例高于对照组(P<0.05);中药饮片使用率均值达95%以上,使用中医非药物疗法比例均值达99%以上,远超满分值(≥60%)。结论在国家公立医院绩效考核新模式下,运用DRG管理工具对三甲中医院康复科开展住院绩效评价,有利于提高康复科住院医疗服务综合水平,促进住院中医药特色优势发挥。Objective To analyse the performance evaluation of rehabilitation department in a tertiary traditional Chinese medicine hospital using diagnosis related groups(DRG)under the national public hospital performance evaluation model.Methods The inpatient medical record homepage information of seven wards in the Department of Rehabilitation,Guangdong Second Traditional Chinese Medicine Hospital before the implementation of DRG from January to December 2022(control group)and after the implementation of DRG from January to December 2023(observation group)were selected,and a total of 10527 cases were grouped by the national version of CN-DRGs,including 4947 cases in the control group and 5580 cases in the observation group.The changes and differences in DRG indicators and traditional Chinese medicine characteristic indicators before and after the implementation of DRG evaluation were compared and analyzed.Results The total weight,case-mix index,and number of difficult and severe cases in the observation group were higher than those in the control group(P<0.05).The time consumption index and cost consumption index of the observation group were lower than those of the control group(P<0.05).The DRG comprehensive ability score of the observation group was higher than that of the control group(P<0.05).The proportion of discharged patients mainly treated with traditional Chinese medicine in the observation group and the proportion of revenue from traditional Chinese medicine decoction pieces to drug revenue were higher than those in the control group(P<0.05).The average usage rate of traditional Chinese medicine decoction pieces was over 95%,and the average proportion of non-pharmacological therapies using traditional Chinese medicine was over 99%,far exceeding the full score value(≥60%).Conclusion Under the new mode of national examination,using DRG management tools to conduct inpatient performance evaluation of the rehabilitation department of a tertiary traditional Chinese medicine hospital is conducive to improving the co
