超声内镜引导细针穿刺抽吸术诊断成人腹膜后神经母细胞瘤1例

摘　　要：神经母细胞瘤(NB)是儿童最常见的实体颅外肿瘤,但在成人中较为少见。本文回顾性分析湖北省襄阳市中心医院收治的1例成人腹膜后NB患者,患者以腰、背部疼痛为主要症状,入院后完善影像学检查提示左上腹多发占位,住院期间进行超声内镜引导细针穿刺抽吸术(EUS-FNA)取得细胞和组织,最终确诊为NB,给予化疗及其他对症治疗,随访肿瘤未见进展。成人腹膜后NB临床表现缺乏特异性,易被误诊、漏诊,确诊多依赖于术后病理学诊断,但腹膜后的手术风险较大。该病例通过EUS-FNA获取细胞和组织进行病理学诊断,对患者机体损伤小、诊断率高、并发症发生率低,为该病的诊治提供参考。Neuroblastoma(NB)is the most common solid extracranial tumor in children,but it is less common in adults.This article retrospectively analyzes a case of adult retroperitoneal NB admitted to Xiangyang Central Hospital in Hubei Province.The patient presentes with pain in the waist and back as the main symptoms.After admission,complete imaging examinations reveales multiple masses in the upper left abdomen.During hospitalization,endoscopic ultrasound-guided fine needle aspiration(EUS-FNA)is performed to obtain cells and tissues,and the final diagnosis is NB.Chemotherapy and other symptomatic treatments are given,and no tumor progression is observed during follow-up.Clinical manifestations of adult retroperitoneal NB lack specificity and are prone to misdiagnosis and missed diagnosis.Diagnosis often relies on postoperative pathological diagnosis,but the surgical risk for retroperitoneal NB is relatively high.This case underwent pat-hological diagnosis by obtaining cells and tissues through EUS-FNA,which results in minimal damage to the patient’s body,high diagnostic rate,and low incidence of complications,providing reference for the diagnosis and treatment of this disease.
