摘　　要：目的:探究经动脉介入栓塞术(TASE)在治疗婴幼儿血管瘤(IH)中的应用价值。方法:选取2020年1月—2022年12月郑州大学附属河南省儿童医院收治的90例血管瘤患儿作为研究对象,采用电脑随机法将患儿分为两组,观察组(n=45)行TASE治疗,对照组(n=45)采用环形缝扎术。观察两组患儿临床疗效、并发症及术后6个月随访情况,比较两组患儿手术前后瘢痕美观度(VSS)评分、血清β2-微球蛋白(β2-MG)、血管内皮生长因子(VEGF)水平。结果:观察组优良率(88.89%)与对照组(100.00%)比较,差异无统计学意义(χ^(2)=3.388,P>0.05);术后1个月,观察组瘢痕VSS各维度评分均低于对照组,差异均有统计学意义(t=7.420、6.543、9.298、6.064,P<0.05);术后1个月,两组患儿血清β2-MG、VEGF水平较术前下降,但观察组高于对照组,差异均有统计学意义(t=2.007、2.001,P<0.05);两组患儿并发症总发生率比较,差异无统计学意义(χ^(2)=1.607,P>0.05)。结论:TASE是IH安全可靠的治疗方案,能提高瘢痕美容效果。Objective:To investigate the application value of transarterial interventional embolization(TASE)in the treatment of infantile hemangioma(IH).Methods:90 infants with hemangioma treated in the hospital from January 2020 to December 2022 were selected as the study objects.The children were divided into two groups by computer randomized method.The observation group(n=45)received TASE treatment,and the control group(n=45)received ring suture.Clinical efficacy,complications and 6 months after operation of the two groups were observed,and the scar beauty(VSS score),serumβ2-microglobulin(Β2-MG)and vascular endothelial growth factor(VEGF)before and after operation were compared.Results:There was no significant difference between 88.89%of observation group and 100.00%of control group(χ^(2)=3.388,P>0.05).1 month after surgery,scar VSS scores in the observation group were lower than those in the control group,with statistically significant difference(t=7.420,6.543,9.298,6.064;P<0.05).1 month after operation,serumβ2-MG and VEGF levels in 2 groups were decreased compared with those before operation,but the observation group was higher than the control group.The difference was statistically significant(t=2.007,2.001;P<0.05).There was no significant difference between the two groups in the total incidence of complications(χ^(2)=1.607,P>0.05).Conclusion:TASE is a safe and reliable treatment for IH,which can significantly enhance the cosmetic effect of scar.
