通督调神针刺法联合常规康复训练在脑梗死后偏身感觉障碍患者中的应用效果

摘　　要：目的:观察通督调神针刺法联合常规康复训练在脑梗死后偏身感觉障碍患者中的应用效果。方法:选取2021年6月至2023年5月该院收治的92例脑梗死后偏身感觉障碍患者进行前瞻性研究,按照随机数字表法分为研究组与对照组各46例。对照组给予常规康复训练,研究组在对照组基础上联合通督调神针刺法治疗,比较两组感觉障碍评定积分表(FLA)评分、Fugl-Meyer运动功能评定表(FMA)评分、脑血流动力学指标[大脑椎动脉、基底动脉、后动脉收缩期峰血速度]水平和血清中枢神经特异性蛋白(S100β)水平。结果:治疗后,两组FLA评分、FMA评分和椎动脉、基底动脉、后动脉收缩期峰血速度等脑血流动力学指标水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组血清S100β水平低于治疗前,且研究组低于对照组,差异均有统计学意义(P<0.05)。结论:通督调神针刺法联合常规康复训练治疗脑梗死后偏身感觉障碍患者可提高FLA评分、FMA评分和脑血流动力学指标水平,降低血清S100β水平,效果优于单纯常规康复训练。Objective:To observe application effects of Tongdu Tiaoshen acupuncture combined with routine rehabilitation training in patients with hemianesthesia after cerebral infarction.Methods:A prospective study was conducted on 92 patients with hemianesthesia after cerebral infarction admitted to this hospital from June 2021 to May 2023.According to the random number table method,they were divided into study group and control group,46 cases in each group.The control group was given routine rehabilitation training,while the study group was treated with selfmade Tongdu Tiaoshen acupuncture on the basis of that of the control group.The scores of sensory disturbance assessment scale(FLA)and Fugl-Meyer motor assessment scale(FMA),and the levels of cerebral hemodynamic indexes[systolic peak blood velocity of cerebral vertebral artery,basilar artery,posterior artery]and serum central nervous specific protein(S100β)were compared between the two groups.Results:After the treatment,the FLA scores,the FMA scores,and the levels of cerebral hemodynamic indexes such as systolic peak blood velocity of cerebral vertebral artery,basilar artery,and posterior artery in the two groups were higher than those before the treatment,those in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of serum S100βin the two groups were lower than those before the treatment,that in the study group was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:Tongdu Tiaoshen acupuncture combined with routine rehabilitation training can improve the FLA scores,the FMA scores and the cerebral hemodynamic index levels,and reduce the serum S100βlevels in the patients with hemianesthesia after cerebral infarction.Moreover,it is superior to simple routine rehabilitation training.
