摘　　要：目的探讨保留乳头乳晕的乳房切除术联合背阔肌肌皮瓣同期乳房重建术治疗乳腺癌的临床疗效及安全性。方法回顾性分析2023年1月至2024年6月在郑州市第二人民医院行保留乳头乳晕的乳房切除术联合背阔肌肌皮瓣同期乳房重建术治疗的47例乳腺癌患者临床资料。统计分析患者皮瓣存活率、手术效果优良率、手术并发症发生率以及术前、术后3个月时的乳腺癌生活质量专用量表评分。结果47例患者手术时间为(192.43±11.87)min,术中出血量为(78.85±8.96)mL。随访结果显示47例患皮瓣均存活,手术效果优良率为93.62%(44/47);术后3个月时,47例患者QLQ-BR53评分由(147.63±11.25)分下降至(68.67±8.57)分,差异有统计学意义(P<0.001);手术并发症发生率为12.77%(6/47)。结论保留乳头乳晕的乳房切除术联合背阔肌肌皮瓣同期乳房重建术治疗乳腺癌临床疗效较好,可有效保障修复效果,提高患者术后生活质量。Objective To investigate the clinical effificacy and safety of Nipple-areola complex-sparing mastectomy combined with latissimus dorsi muscle flflap breast reconstruction in the treatment of breast cancer.Methods The clinical data of 47 patients with breast cancer who underwent Nipple-areola complex-sparing mastectomy combined with latissimus dorsi muscle flflap simultaneous breast reconstruction in our hospital from January 2023 to June 2024 were retrospectively analyzed.The survival rate of skin flflap,the excellent and good rate of surgical results,the incidence of surgical complications and the scores of quality of life instruments for cancer patients-breast cancer before and 3 months after surgery were statistically analyzed.Results The operative time of 47 patients was(192.43±11.87)min,and the intraoperative blood loss was(78.85±8.96)mL.The follow-up showed that all 47 patients with flflap were alive,and the rate of excellent and good operation was 93.62%(44/47).The QLQ-BR53 score of 47 patients ranged from(147.63±11.25)points to(68.67±8.57)points at 3 months after operation,with statistical signifificance(P<0.001).The complication rate was 12.77%(6/47).Conclusions Nipple-areola complex-sparing mastectomy combined with latissimus dorsi myocutaneous flflap simultaneous breast reconstruction has good clinical effificacy in the treatment of breast cancer,which can effectively guarantee the repair effect and improve the postoperative quality of life of patients.
