前房穿刺术联合虹膜周切治疗急性发作闭角型青光眼患者的临床效果

摘　　要：目的研究前房穿刺术联合虹膜周切治疗急性发作闭角型青光眼患者的临床效果。方法选取2021年6月至2023年6月我院收治的64例急性发作闭角型青光眼患者,根据治疗方案不同将其分为对照组和观察组,各32例。对照组采用常规治疗,观察组采用前房穿刺术联合虹膜周切治疗。比较两组的治疗效果。结果治疗后,观察组的视力高于对照组,眼压低于对照组(P<0.05)。治疗后,观察组的患眼角膜内皮细胞数量多于对照组(P<0.05)。治疗后,观察组的中央前房深度(CACD)、周边前房深度(PACD)及房角宽度(ACAW)高于对照组(P<0.05)。治疗后,观察组的超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论前房穿刺术联合虹膜周切治疗急性发作闭角型青光眼患者的效果确切,可调节眼压及视力,改善房角宽度,降低炎症反应程度及并发症发生率,值得推广。Objective To study the clinical effect of anterior chamber puncture surgery combined with peripheral iridectomy in the treatment of patients with acute attack of angle-closure glaucoma.Methods A total of 64 patients with acute attack of angle-closure glaucoma admitted in our hospital from June 2021 to June 2023 were selected and divided into control group and observation group according to different treatment schemes,with 32 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with anterior chamber puncture surgery combined with peripheral iridectomy.The therapeutic effects of the two groups were compared.Results After treatment,the vision in the observation group was higher than that in the control group,and the intraocular pressure was lower than that in the control group(P<0.05).After treatment,the number of corneal endothelial cells in the observation group was more than that in the control group(P<0.05).After treatment,the central anterior chamber depth(CACD),peripheral anterior chamber depth(PACD)and anterior chamber angle width(ACAW)in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of hypersensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Anterior chamber puncture surgery combined with peripheral iridectomy is effective in the treatment of patients with acute attack of angle-closure glaucoma,it can regulate intraocular pressure and vision,improve the width of the angle,and reduce the degree of inflammation and incidence of complications,which is worth promoting.
