经ERCP置入胆管支架与姑息性手术治疗晚期肝外胆管癌的疗效分析

摘　　要：目的分析经内镜逆行胰胆管造影术(ERCP)置入胆管支架与姑息性手术(PS)治疗晚期肝外胆管癌(EHCC)的疗效。方法选择经ERCP置入胆管支架(ERCP组,51例)或姑息性手术(PS组,接受胆管空肠吻合联合胆囊切除术,42例)治疗的EHCC的患者。分析两组患者术前以及术后第5天肝功能的变化;比较两组患者住院时间、费用、并发症、总体生存时间的差异。结果两组术后第5天肝功能有不同程度的改善。PS组的术后减黄效果更佳,肝功能改善更明显,但是住院时间较长,住院费用较高,术后出现的并发症较多,术后生存时间明显小于ERCP组。结论与PS术相比,经ERCP胆管支架置入可缩短晚期EHCC患者的住院时间,减少住院费用,提高生存时间,降低并发症发生率。Objective To evaluate the efficacy of endoscopic retrograde cholangiography(ERCP)with bile duct stents and palliative surgery(PS)in the treatment of advanced extrahepatic cholangiocarcinoma(EHCC).Methods Patients with advanced extrahepatic cholangiocarcinoma were selected for ERCP implantation of bile duct stents(ERCP group,51 cases)or palliative surgery(PS group,42 cases).The changes of liver function before operation and on the 5th day after operation were analyzed.The differences of hospital stay,cost,complications and overall survival time were compared between the two groups.Results The liver function of the two groups was improved in different degree on the 5th day after operation.PS group had better postoperative yellow reduction effect and more obvious improvement of liver function,but the hospital stay was longer,the hospitalization cost was higher,the postoperative complications were more,and the postoperative survival time was significantly lower than that of ERCP group.Conclusion Compared with PS,biliary stent implantation via ERCP can shorten hospital stay,reduce hospital cost,increase survival time and reduce complication rate in advanced EHCC.
