摘　　要：目的 分析不同病理类型孤立性肺癌患者的多层螺旋CT(MSCT)影像学特征。方法 选取52例孤立性肺癌患者,根据病理类型的不同分为腺癌组(n=34)、鳞状细胞癌组(n=12)和小细胞癌组(n=6)。所有患者均进行MSCT检查,比较3组患者的CT值、强化情况、强化方式、相对廓清值及廓清率。结果 MSCT增强扫描30、60 s时,腺癌组患者CT值均高于鳞状细胞癌组和小细胞癌组,差异均有统计学意义(P﹤0.05);MSCT增强扫描120 s时,腺癌组和鳞状细胞癌组患者CT值均高于小细胞癌组,差异均有统计学意义(P﹤0.05)。腺癌组、鳞状细胞癌组患者的强化净增值均高于小细胞癌组,差异均有统计学意义(P﹤0.05)。腺癌组患者均匀强化率高于鳞状细胞癌组和小细胞癌组,差异均有统计学意义(P﹤0.05)。MSCT延迟扫描180 s时,小细胞癌组患者相对廓清值、廓清率均高于腺癌组和鳞状细胞癌组,差异均有统计学意义(P﹤0.05);MSCT延迟扫描600 s时,腺癌组患者相对廓清值、廓清率均低于鳞状细胞癌组和小细胞癌组,差异均有统计学意义(P﹤0.05)。结论 不同病理类型孤立性肺癌患者的MSCT影像学特征存在明显差异,临床可综合评估不同扫描时间CT值、强化情况等多种影像学特征,为孤立性肺癌患者病理类型的诊断提供参考依据。Objective To analyze the multi-slice spiral CT(MSCT)imaging features in patients with solitary lung cancer of different pathological types.Method Fifty-two patients with solitary lung cancer were selected and divided into adenocarcinoma group(n=34),squamous cell carcinoma group(n=12)and small cell carcinoma group(n=6)according to different pathological types.All patients underwent MSCT examination,and the CT value,enhancement condition,enhancement mode,relative clearance value and clearance rate of the three groups were compared.Result At 30 and 60 s of enhanced MSCT scanning,the CT values in adenocarcinoma group were higher than those in squamous cell carcinoma group and small cell carcinoma group,and the differences were statistically significant(P<0.05).At 120 s of enhanced MSCT scanning,the CT values in adenocarcinoma group and squamous cell carcinoma group were higher than that in small cell carcinoma group,and the differences were statistically significant(P<0.05).The enhanced net increment in ade-nocarcinoma group and squamous cell carcinoma group were higher than that in small cell carcinoma group,and the differences were statistically significant(P<0.05).The uniform enhanced rate in adenocarcinoma group was higher than those in squamous cell carcinoma group and small cell carcinoma group,and the differences were statistically significant(P<0.05).At 180 s of delayed MSCT scanning,the relative clearance value and clearance rate of small cell carcinoma group were higher than those of adenocarcinoma group and squamous cell carcinoma group,and the differences were statistically significant(P<0.05).At 600 s of delayed MSCT scanning,the relative clearance value and clearance rate of ade-nocarcinoma group were lower than those of squamous cell carcinoma group and small cell carcinoma group,and the differences were statistically significant(P<0.05).Conclusion There are significant differences in MSCT imaging features of patients with solitary lung cancer of different pathological types.Clinical comprehensive eval
