摘　　要：目的:探究肿瘤细胞减灭术联合腹腔热灌注化疗治疗晚期(FIGO 2018Ⅲ~Ⅳ期)上皮性卵巢癌的效果及安全性。方法:回顾分析2018年1月至2023年1月于首都医科大学附属北京妇产医院诊疗的127例晚期上皮性卵巢癌患者的临床资料。患者均予以肿瘤细胞减灭术和术后常规化疗,其中51例术后联合腹腔热灌注化疗(HIPEC)(研究组),76例术后未行HIPEC(对照组)。比较两组患者的住院时间、并发症发生情况以及无疾病进展生存期(PFS)和总生存期(OS)。结果:对照组患者的OS有短于研究组的趋势,但差异无统计学意义(P=0.105),PFS明显短于研究组,差异有统计学意义(P=0.019)。研究组术后2、3年复发率明显低于对照组(P<0.05)。两组的住院时间及术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:肿瘤细胞减灭术联合腹腔热灌注化疗治疗晚期(FIGO 2018年Ⅲ~Ⅳ期)上皮性卵巢癌能明显延长PFS,降低复发率,且不会增加患者住院时间及术后并发症的发生率。Objective:To investigate the efficacy and safety of cytoreduction surgery combined with hyperthermic intraperitoneal chemotherapy on patients with advanced(FIGO 2018 stageⅢ~Ⅳ)epithelial ovarian cancer.Methods:The clinical data of 169 patients with advanced epithelial ovarian cancer treated at Beijing Maternity Hospital of Capital Medical University from January 2018 to January 2023 were retrospectively analyzed.All patients received cytoreductive surgery and conventional chemotherapy,and 51 patients were given hyperthermic intraperitoneal chemotherapy after surgery(study group),76 patients were not given hyperthermic intraperitoneal chemotherapy after surgery.The hospitalization time,complications,the progression-free survival(PFS)and overall survival(OS)of the two groups were compared.Results:Patients in the control group had a significantly shorter PFS compared to the study group(P=0.019).There was no statistical difference between the two groups for OS.The 2 and 3-year recurrence rates in the study group were significantly better than those in the control group(P<0.05).There was no statistically significant difference in hospital stay and incidence of postoperative complications between the two groups(P>0.05).Conclusions:Cytoreduction surgery combined with hyperthermic intraperitoneal chemotherapy would significantly improve the PFS,reduce the recurrence rates of advanced epithelial ovarian cancer patients,and will not increase the hospital stay and the incidence of postoperative complications.
