复方黄硝散外敷神阙穴对胃癌术后早期肠内营养患者预后及营养状况的影响

摘　　要：目的观察复方黄硝散外敷神阙穴对胃癌术后早期肠内营养耐受性的影响。方法选取2020年7月至2021年6月湖北医药学院附属人民医院胃肠外科胃癌术后行早期肠内营养治疗的80例患者作为研究对象,按照入院时间分为2组,对照组40例予胃癌术后肠内营养常规管理,治疗组40例在对照组基础上加复方黄硝散持续外敷神阙穴,直至患者完全恢复经口正常进食。比较2组首次肛门排气时间、首次排便时间、肠鸣音恢复时间、空肠营养管留置时间及肠内营养耐受性情况,比较2组治疗前后营养指标(前白蛋白、白蛋白、总蛋白)和感染指标[C反应蛋白(CRP)和降钙素原(PCT)]水平变化。结果治疗组首次肛门排气时间、首次排便时间、肠鸣音恢复时间及空肠营养管留置时间均较对照组提前(P<0.05)。治疗组肠内营养不耐受发生率10.00%(4/40),对照组肠内营养不耐受发生率30.00%(12/40),治疗组低于对照组(P<0.05)。治疗后2组前白蛋白、白蛋白、总蛋白水平均较本组治疗前升高(P<0.05),且治疗组前白蛋白、白蛋白水平均高于对照组(P<0.05)。治疗后2组PCT、CRP水平均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。结论复方黄硝散外敷神阙穴可明显缩短胃癌术后早期肠内营养患者胃肠功能恢复时间,改善患者术后行早期肠内营养不耐受性现象和营养状况,控制感染。Objective To investigate the effect of external application with compound Huangxiao Powder at Shenque(RN8)for early enteral nutrition(EEN)tolerance in gastric cancer(GC)patients after surgery.Methods Eighty GC patients undergoing EEN after surgery treated in our Department of Gastrointestinal Surgery,People's Hospital Affiliated to Hubei University of Medicine from July 2020 to June 2021 were enrolled.They were assigned into treatment group(n=40)and control group(n=40)according to the admission time.All patients were managed by routine enteral nutrition,and those in observation group additionally received compound Huangxiao Powder at Shenque(RN8)until the complete recovery of normal oral feeding.The first exhausting time of anus,first defecation time,bowel sounds recovery time,indwelling time of jejunum tube,enteral nutrition tolerance,nutritive indexes(prealbumin[PA],albumin,total protein[TP]),infection indexes(C-reactive protein[CRP],procalcitonin[PCT])were compared between groups.Results The first exhausting time of anus,first defecation time,bowel sounds recovery time,indwelling time of jejunum tube were significantly shorter in the treatment group than in the control group(P<0.05).The incidence of enteral nutrition intolerance in the treatment group was significantly lower than that in the control group(10.00%[4/40]vs 30.00%[12/40],P<0.05).After treatment,PA,albumin,TP in the both groups were significantly increased(P<0.05),PA and albumin levels in the treatment group were higher than those in control group(P<0.05).PCT and CRP levels in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more common than the control group(P<0.05).Conclusion For GC patients undergoing EEN after surgery,external application with compound Huangxiao Powder at Shenque(RN8)can significantly shorten the recovery time of gastrointestinal function,improve EEN intolerance and nutritional status,and control infection.
