摘  要:目的探究磁共振三维动脉自旋标记(3D-ASL)脑灌注成像技术在缺血性脑血管疾病中的应用价值。方法选择2021年7月—2023年9月本院收治的疑似缺血性脑血管疾病患者80例作为研究对象,均采用3D-ASL、数字减影血管造影(DSA)、磁共振血管成像(MRA)检查。以DSA检查结果为“金标准”,分析MRA、3D-ASL检查结果,另分析MRA、3D-ASL在缺血性脑血管疾病中的诊断价值,并计算与DSA结果的一致性。结果经DSA检查明确缺血性脑血管疾病患者共58例,MRA检查共检出57例,3D-ASL检查检出59例。相比于MRA检查,3D-ASL检查在缺血性脑血管疾病诊断中的灵敏度、准确度均较高,差异有统计学意义(P<0.05),特异度、阴性及阳性预测值不同检查方法对比,差异无统计学意义(P>0.05);一致性检验显示,3D-ASL诊断与DSA结果一致性良好(kappa=0.841,P=0.000),MRA诊断与DSA结果一致性尚可(kappa=0.536,P=0.000)。结论3D-ASL脑灌注成像技术在缺血性脑血管疾病鉴别诊断中具有较高的准确度,且与DSA结果一致性较强,可将其作为临床鉴别诊断的首选方法,值得推广应用。Objective To explore the application value of magnetic resonance three-dimensional arterial spin labeling(3D-ASL)cerebral perfusion imaging technology in ischemic cerebrovascular diseases.Methods Eighty suspected ischemic cerebrovascular disease patients admitted to our hospital from July 2021 to September 2023 were included in the study,all of whom underwent 3D-ASL,digital subtraction angiography(DSA),and magnetic resonance angiography(MRA)examinations.Using DSA examination results as the"gold standard,"the examination results of MRA and 3D-ASL were analyzed,and the diagnostic value of MRA and 3D-ASL in ischemic cerebrovascular disease was also analyzed,calculating the consistency with DSA results.Results The results showed that there were 58 patients with ischemic cerebrovascular disease confirmed by DSA examination,57 cases detected by MRA examination,and 59 cases detected by 3D-ASL examination.Compared with MRA examination,3D-ASL examination has higher sensitivity and accuracy in the diagnosis of ischemic cerebrovascular disease,with statistically significant differences(P<0.05).There was no statistical difference in specificity,negative predictive value,and positive predictive value among different examination methods(P>0.05).The consistency test showed that the 3D-ASL diagnosis had good consistency with DSA results(kappa=0.841,P=0.000),while the consistency between MRA diagnosis and DSA results was still acceptable(kappa=0.536,P=0.000).Conclusions 3D-ASL cerebral perfusion imaging technology has high accuracy in the differential diagnosis of ischemic cerebrovascular diseases and strong consistency with DSA results.It can be used as the preferred method for clinical differential diagnosis and is worthy of promotion and application.
