基于程序的整体干预在胸腔镜手术治疗老年肺癌中的运用效果

摘　　要：目的 探讨基于程序的整体干预在胸腔镜手术治疗老年肺癌中的运用效果。方法 将2021年1月—2023年1月就诊于河南省胸科医院的80例行胸腔镜手术治疗的老年肺癌患者采用不同干预方法对患者进行随机的分组,分为常规组40例、整体干预组40例。常规组的研究对象行常规干预,整体干预组研究对象采用程序的整体干预进行干预,对比2组干预前与干预后肺功能指标、呼吸困难量表、生活态度、并发症及满意度的变化。结果 术后与常规组FEV_(1)(2.11±0.12)L、FEV_(1)/FVC(29.88±2.79)%水平及HERTH评分(25.21±2.56)分、分钟通气(5.10±0.46)L/Kg、肺的顺应性(46.77±4.61)mL/cmH_(2)O、呼吸功(0.06±0.04)J/L相比,整体干预组FEV_(1)(3.01±0.34)L、FEV_(1)/FVC(41.22±4.23)%水平及HERTH评分(35.19±3.55)分、分钟通气(6.96±0.71)L/Kg、肺的顺应性(49.91±4.80)mL/cmH_(2)O、呼吸功(0.11±0.12)J/L明显升高;且与常规组呼吸困难量表(1.23±0.16)分、恐惧情绪自测表(20.91±2.12)分及并发症相比,整体干预组呼吸困难量表(0.76±0.07)分、恐惧情绪自测表(11.03±1.08)分明显下降,满意度提升,具有统计学意义(P<0.05)。结论 通过程序的整体干预的开展,能有效改善胸腔镜手术治疗的老年肺癌患者肺功能,改善患者生活态度。Objective To explore the effect of procedure-based holistic intervention in thoracoscopic surgery for elderly lung cancer.Methods Eighty elderly lung cancer patients who visited our hospital from January 2021 to January 2023 were randomized by different intervention methods and divided into routine group 40 cases and overall intervention group 40 cases.The study subjects of the routine group performed routine intervention,and the overall intervention group used the overall intervention to compare the changes of lung function indicators,dyspnea scale,life attitude,complications and satisfaction before and after the intervention in the two groups.Results Compared with routine group FEV_(1)(2.11±0.12)L,FEV_(1)/FVC(29.88±2.79)%level and HERTH score(25.21±2.56),minute ventilation(5.10±0.46)L/KG,lung compliance(46.77±4.61)mL/cm H2 O,breathing work(0.06±0.04)J/L,the overall intervention group FEV_(1)(3.01±0.34)L,FEV_(1)/FVC(41.22±4.23)%level and HERTH score(35.19±3.55)points,minute ventilation(6.96±0.71)L/KG,lung compliance(49.91±4.80)mL/cmH2 O,breathing work(0.11±0.12)J/L increased obviously;And compared with the routine group dyspnea scale(1.23±0.16),fear self-test table(20.91±2.12)and complications,the overall intervention group,the dyspnea scale(0.76±0.07)points and the fear mood self-test table(11.03±1.08)points decreased significantly,Satisfaction improved obviously with significant difference(P<0.05).Conclusion Through the overall intervention program,the lung function of elderly lung cancer patients can effectively improve the life attitude treated by thoracoscopic surgery.
