摘 要:目的探讨部分脾动脉栓塞术(PSE)后氧化应激系统改变对肝功能的影响。方法选取接受PSE治疗的肝硬化伴脾功能亢进患者29例。于术前及术后1周、1月、3月抽取患者外周静脉血,检测氧化应激系统因子:丙二醛(MDA)、超氧化物歧化酶(SOD)、晚期氧化蛋白产物(AOPPs)、谷胱甘肽过氧化物酶(GSH-Px)的含量,以及肝功能指标。结果术后1周SOD活力与总胆红素(TBil)、终末期肝病模型(MELD)评分呈正相关(TBil:r=0.725,P<0.05;MELD:r=0.764,P<0.05);术后1月GSH-Px活力与丙氨酸转氨酶(ALT)含量呈正相关(r=0.777,P<0.05);术后3月AOPPs与ALT、天门冬氨酸氨基转移酶(AST)呈负相关(ALT:r=-0.900,P<0.05;AST:r=-0.957,P<0.05)。结论PSE能改善机体氧化应激系统,增强机体抗氧化反应,改善肝功能。Objective To investigate the effect of altered oxidative stress system on liver function after partial splenic embolization(PSE).Methods Twenty-nine patients with liver cirrhosis and hypersplenism who received PSE were selected.Peripheral venous blood was drawn from the patients before and at 1 week,1 month,and 3 months after PSE,and the indexes of oxidative stress system factors including malondialdehyde(MDA),superoxide dismutase(SOD),advanced oxidiation protein products(AOPPs),and glutathione peroxidase(GSH-Px)were detected,as well as liver function indexes.Results There were positive correlation between SOD activity and total bilirubin(TBil)and model for end-stage liver disease(MELD)scores at 1 week postoperatively(TBil:r=0.725,P<0.05;MELD:r=0.764,P<0.05).There was positive correlation between GSH-Px activity and alanine aminotransferase(ALT)at 1 month postoperatively(r=0.777,P<0.05),however,the AOPPs was negatively correlated with ALT and aspartate aminotransferase(AST)at 3 months postoperatively(ALT:r=-0.900,P<0.05;AST:r=-0.957,P<0.05).Conclusion PSE can improve the body oxidative stress system and enhance the body’s antioxidant response,and then improve the liver function.
