摘 要:目的探讨沉默信息调节因子1(SIRT1)和缺氧诱导因子(HIF)-1α在皮肤恶性黑色素瘤(CMM)中的表达及其与临床因素之间的关系。方法收集经组织病理和免疫组化明确诊断为CMM和黑色素痣的手术切除标本各30例,采用RT-qPCR和免疫组化方法检测SIRT1和HIF-1α在CMM和黑色素痣组织样本中的表达量,分析CMM组织中SIRT1和HIF-1α的表达与患者的性别、年龄、病变部位、病理类型、Clark病理分级、淋巴结转移情况、肿瘤最大直径之间的相关性。结果RT-qPCR检测结果显示,SIRT1 mRNA在黑色素痣中相对表达量低于在CMM中的相对表达量(P<0.01);HIF-1αmRNA在黑色素痣中的相对表达量低于在CMM中的相对表达量(P<0.05);免疫组化检测结果显示:与黑色素痣相比,SIRT1在CMM中的表达明显增强(t=10.14,P<0.001),HIF-1α在CMM中的表达增强(t=21.99,P<0.001)。SIRT1的表达与肿瘤的Clark分级及淋巴结转移有关(P均<0.05);HIF-1α的表达与肿瘤的Clark分级、发病部位及淋巴结转移有关(P均<0.05)。结论CMM患者肿瘤组织中SIRT1和HIF-1α阳性表达率升高,且与Clark分级、淋巴结转移有关,HIF-1α的表达还与肿瘤发病部位有关,SIRT1和HIF-1α可能成为预测肿瘤侵袭、转移及预后的生物标志物。Objective To investigate the expression of SIRT1 and HIF-1αin cutaneous malignant melanoma(CMM)and analyze their relationship with clinical factors.Methods A total of 30 surgical specimens of CMM and melanocytic nevus confirmed by histopathology and immuno-histochemistry were collected from the Department of Dermatology of General Hospital of Ningxia Medical University from February 2022 to February 2024.The expressions of SIRT1 and HIF-1αwere detected by RT-qPCR and immunohistochemical method in CMM and melanocytic nevus tissue samples.Chi-square test was used to analyze and evaluate the correlation between the expression of SIRT1 and HIF-1αand the gender,age,tumor site,tumor lesion diameter and pathological grade of patients.Results RT-qPCR results showed that the relative expression level of SIRT1 mRNA in melanocytic nevus was lower than that of CMM(P<0.01).The relative expression level of HIF-1αmRNA in melanocytic nevus was lower than that of CMM(P<0.05).Immunohistochemical experiment results showed that the expression of SIRT1 in CMM was significant high than that in melanocytic nevus(t=10.14,P<0.001),and the expression of HIF-lαincreased in CMM,which was significant high than that in melanocytic nevus(t=21.99,P<0.001).The expression intensity of SIRT1 was correlated with tumor Clark grade and lymph node metastasis(P all<0.05).The expression intensity of HIF-1αwas correlated with tumor grade,site and lymph node metastasis(P all<0.05).Conclusion The expression of SIRT1 and HIF-1αin CMM tissues were increased,and they were related to Clark grade,lymph node metastasis,and tumour site,which could be used as biomarks to predict tumor invasion,metastasis and prognosis.
