摘 要:目的:分析急性髓系白血病(AML)患者临床特点及微移植治疗的效果。方法:回顾性分析2019年1月—2022年6月于中山市人民医院进行微移植治疗的9例老年AML患者的临床资料。结果:9例AML患者疾病类型多为急性单核细胞性白血病;染色体核型正常5例,异常4例;分子学异常类型包括FLT3-ITD基因突变、AML-ETO融合基因阳性、增强子结合蛋白基因突变等;东部肿瘤协作组评分多为1分。患者白细胞水平恢复正常时间为11(10,14)d,血小板水平恢复正常时间为17(15,20)d;有1例患者治疗后因并发肺部感染死亡,完全缓解7例,复发1例;所有患者微移植治疗后未发生移植物抗宿主病(GVHD)。患者治疗后均发生不良反应。结论:AML的发生与分子生物学异常有关,治疗难度大,患者不良反应发生风险以及复发风险高,微移植治疗可避免患者发生GVHD,有望成为治疗难治、复发AML的新方法。Objective:To analyze the clinical characteristics of patients with acute myeloid leukemia(AML)and efficacy of microtransplantation treatment.Methods:The clinical data of 9 elderly patients with AML who underwent microtransplantation in Zhongshan City People's Hospital from January 2019 to June 2022 were retrospectively analyzed.Results:Most of the 9 AML patients had acute monocytic leukemia;There were 5 cases with normal chromosomal karyotype and 4 cases with abnormal chromosomal karyotype;The types of molecular abnormalities included FLT3-ITD gene mutation,AML-ETO fusion gene positive,and enhancer-binding protein gene mutation;The Eastern Cooperative Oncology Group score was mainly 1 point.The time for returning to normal white blood cell count was 11(10,14)d,and the time for returning to normal platelet count was 17(15,20)d;1 patient died of concurrent pulmonary infection after treatment,7 patients had complete remission and 1 patient had recurrence;All patients did not develop graft-versus-host disease(GVHD)after microtransplantation treatment.All patients had adverse reactions after treatment.Conclusion:The occurrence of AML is related to molecular biological abnormalities,which is difficult to treat,high risk of adverse reactions and high risk of recurrence.Microtransplantation treatment can avoid GVHD in patients,which is expected to become a new approach for the treatment of refractory and recurrent AML.
