光学相干断层扫描血管成像联合视网膜地形图对复杂性视网膜脱离黄斑区的影响

摘　　要：目的探讨光学相干断层扫描血管成像(OCTA)联合视网膜地形图对复杂性视网膜脱离黄斑区的影响。方法选择2023年1月至2024年4月医院确诊的120例复杂性视网膜脱离的患者为研究对象,其中120只健侧眼设为对照组,另120只患侧眼设为试验组。两组均应用OCTA联合视网膜地形图,比较两组眼球视网膜浅层毛细血管丛血管密度、视网膜深层毛细血管丛血管密度、黄斑区视网膜厚度、黄斑区内外脉络膜厚度。结果试验组内环上方(IS)、内环鼻侧(IN)、内环下方(OS)和外环鼻侧(ON)视网膜浅层毛细血管丛血管密度低于对照组,差异有统计学意义(P<0.05);试验组黄斑区中心凹区域(C)、IN、内环颞侧(IT)、IS、内环下方(II)、ON、外环颞侧(OT)、OS、外环下方(OI)内、外层视网膜厚度及脉络膜厚度分别高于对照组,差异有统计学意义(P<0.05)。结论OCTA联合视网膜地形图显示IN、IS、OS、ON区域视网膜浅层毛细血管丛血管密度下降,C、IN、IT、IS、II、ON、OT、OS、OI区域内、外视网膜厚度及外脉络膜厚度增厚,可以作为诊断复杂性视网膜脱离的重要指标。Objective To explore the impalt of optical coherence tomography angiography combined with retinal topography on the macular area of complex retinal detachment.Methods A total 120 patients with complex retinal detachment diagnosed in hospitals from January 2023 to April 2024 were selected.Among them,120 affected eyes were set as the experimental group,and 120 healthy eyes were set as the control group.OCTA combined with retinal topography was used to compare the changes in vascular density,choroidal thickness,and retinal thickness of the superficial and deep capillary plexuses in the affected and healthy eyes.Results The vascular density of the capillary plexus shallow retinal layers in the IS,IN,OS,and ON regions of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05);The thickness of the retinal thickness and choroidal thickness in the macular area C,IN,IT,IS,II,ON,OS,OT,OI of the experimental group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusions OCTA combined with retinal topographic map shows a decrease in vascular density of the superficial retinal capillary plexus in the IN,IS,OS,and ON regions,while thickening of the outer retina thickness and outer choroid thickness in the C,IN,IT,IS,II,ON,OT,OS,and OI regions can be important indicators for diagnosing complex retinal detachment.
